On November 10th, 2023 time 09:42 AM while BVI-
HSE Performed daily site inspection, found unsafe
condition the protruding rebar was not cut immediately,
potentially causing someone to be slashed by the rebar
Location: Outbound 2nd floor

Description: Description :
On November 10th, 2023 time 09:43 AM while BVI-
HSE Performed daily site inspection, found unsafe dismantled formwork materials have been taken
condition the dismantled formwork material was not down
taken down immediately, potentially fall down and hit
the workers below

condition many piles of garbage were not removed
