    On October 30th, 2023, BVI-HSE performed a daily site inspection and found several unsafe conditions and actions. These included a cat ladder platform that was missing a safe working load sign, rubbish and unused materials left on a steel structure, power cables improperly arranged and lying on the floor in front of an emergency exit, workers installing power cables standing on a cable ladder without scaffolding, and a broken glass lamp that was not cleaned up. PT. Pulau Intan was required to improve these findings and provide evidence of corrective actions taken.
