QUESTIONING TOOL
Extension/Reflection Questions
1. In our daily pharmacy practice, We prioritize patient safety by double-
checking medication orders, verifying patient information, and reviewing
medication profiles for potential drug interactions. We also counsel patients
on proper medication use and potential side effects to minimize any risks.
2. To stay updated, We need to regularly attend continuing education
programs, participate in professional organizations, and read industry
journals. We also need to engage in discussions with colleagues and actively
seek out new information through online platforms and conferences
3. Communication is vital for optimal patient outcomes. We need to ensure
clear and concise communication by actively listening to patients' concerns,
providing accurate information, and using appropriate language to convey
instructions.
4. We need to actively contribute to the improvement of pharmacy services and
patient care by actively participating in quality improvement initiatives,
suggesting process enhancements, and implementing evidence-based
practices.
Safety Questions
5. Implementing safety protocols in practicing pharmacy services is crucial to
ensure patient well-being and minimize potential risks. Failure to follow
these protocols can lead to serious consequences such as medication errors,
adverse drug reactions, or even harm to patients. For example, incorrect
dosage calculations or dispensing errors can result in patients receiving the
wrong medication or incorrect dosages, leading to adverse effects or
treatment failure.
6. Effective communication plays a vital role in maintaining a safe environment
during pharmacy services. It ensures accurate information exchange between
healthcare professionals, patients, and other stakeholders. Miscommunication
can lead to serious safety hazards, such as medication errors or drug
interactions. For instance, if a pharmacist fails to communicate essential drug
information to a patient, they may unknowingly take medications that interact
negatively with each other, resulting in harmful side effects.
7. Medication reconciliation involves comparing a patient's current medication
regimen with any new medications prescribed during their healthcare journey.
The steps include obtaining a comprehensive medication history, verifying the
accuracy of the information, reconciling discrepancies, and communicating the
updated medication list to the patient and other healthcare providers involved.
This process is crucial for patient safety as it helps prevent medication errors,
duplications, and potential interactions between medications.
Note: In the remarks section, remarks may include for repair, for
replenishment, for reproduction, for maintenance etc.
