follows:

1. I say that I am the deponent herein.

2. I say that I have approached loan from Central Bank of

India, Branch, for Rs. 75,000/- (Rupees Seventy
Five Thousand).

3. I say that I am Contractor by Profession earning annual

income is Rs. 1,02,500/-.

4. I say that I have applied for PAN Card and filed Tax returns
to concerned Tax department.

5. I say that I will undertake to produce said PAN Card and

returns after receiving the same from concerned Tax
Department.
