In partial fulfilment of the requirement stated in the curriculum of the Bachelor of Science in Civil
Engineering and Environmental and Sanitary Engineering program, all candidates are required to
undergo at least 240 hours on-the-job training (OJT/Practicum). The objective of the said OJT/Practicum
is to give him/her the opportunity to apply his acquired knowledge to actual work situation.
Furthermore, this is regarded toward gaining experience or exposure to different pertinent practices.

In line with this, I would like to request your office to allow Mr. Joshua Rich G. Bitancor, Gabriel
Mathew S.B. Sampayo and Leo Justin C. Bayaoa. Any favor accorded would be highly appreciated and
would mean that your company is one with us in helping to strengthen quality education. Likewise, we
wish to invite your company to join the roster of companies who have already signed a Memorandum of
Agreement with Mapúa to bolster OJT program and other career-related services we offer to our
students and alumni. Rest assured that all information gathered will be treated with utmost
confidentiality and will be used for classroom discussion only.

Enclosed herewith is a copy of the trainee’s resume for your perusal and his/her available schedule for
OJT/practicum.

Should you consider our request, kindly accomplish the acceptance form attached to this letter.
Likewise, a student training agreement & liability waiver issued by the legal office of Mapúa will be
forwarded to you upon approval.
