I/WE, MARILOU ESCOBIA ZULUETA and ALEXANDER GILLADO ZULUETA of

legal age, Filipino/s, and with office/home address at PUROK 4B POBLACION MAASIM
MAASIM SARANGANI, after being sworn to in accordance with law hereby depose and say
that:
1. I/We am/are the borrower/lessee of Toyota Financial Services Philippines
Corporation (TFSPH) as evidenced by a Promissory Note with Chattel Mortgage/Contract
of Lease/Motor Vehicle Rental Agreement, over a motor vehicle specifically described as
follows:

3. I/We hereby undertake/s not to use the above-described motor vehicle for
any unlawful act/s and/or to sell, alienate, mortgage, pledge or dispose the same,
nor to allow myself/ourselves, directly or indirectly, to use my/our credit approval
by TFSPH or be a dummy of any person/s or entity/ties;
4. Failure on my/our part to comply with the above-mentioned undertaking, I/We
hereby obligated myself/ourselves to immediately pay, without the necessity of demand, the
amount of the outstanding balance of my/our loan/indebtedness/liquidated damages to
Toyota Financial Services Philippines Corporation (TFSPH) plus interest, charges and/or
expenses as provided for under the Promissory Note/Contract of Lease/Motor Vehicle
Rental Agreement and/or be held criminally liable for estafa and/or violation of chattel
mortgage punishable under the Revised Penal Code (RPC), and/or any other related law/s.
5. That I/We execute this Affidavit of Undertaking to attest to the truth of the
foregoing, aware of my/our responsibilities in case of falsity and/or failure to comply
thereof.
