Corporation (TFSPH) as evidenced by a Promissory Note with Chattel Mortgage/ Contract
of Lease, with contract value of P 745.600 for a period of 60 months commencing on

with TFSPH has already been fully settled;
4. Failure on my/our part to comply with the above-mentioned undertaking, I/We
hereby obligated myself/ourselves to immediately pay, without the necessity of demand,
the amount of the outstanding balance of my/our loan/indebtedness to Toyota Financial
Services Philippines Corporation (TFSPH) plus interest, charges and/or expenses as
provided for under the Promissory Note/Contract of Lease;
5. That I/We execute this Affidavit of Undertaking to attest to the truth of the
foregoing aware of my/our responsibilities in case of falsity and failure to comply thereof.
