ABSOLUTE WAIVER

I/We, HELEN ALMONICAR GALLO of 4TH STREET LACAP SUBD. PRK MALAKAS GENERAL SANTOS CITY SOUTH COTABATO located at National highway City
Heights General Santos City, hereinafter referred to as the Dealer, absolutely free from any liability of whatsoever nature that may arise out of my request to release the vehicle that I have
purchased from said Dealer, more particularly described as follows:

That for and in consideration of the release of the foregoing motor vehicle to me via a car carrier or towing truck, or driven by me or my representative, or by a driver of the Dealer, from
the Dealer to my address 4TH STREET LACAP SUBD. PRK MALAKAS GENERAL SANTOS CITY SOUTH COTABATO hereby unconditionally hold TOYOTA GENERAL
SANTOS, INC upon my request, without its PNP Clearance, Certificate Stock Report (CSR) and without having been registered with the Land Transportation Office (LTO), I do hereby
release and forever hold free, the Dealer and its successors-in-interest, from any liability, criminal, civil, and administrative, in connection with said immediate release of the vehicle.

This is also to confirm that I have been warned by the Dealer not to use my vehicle purchased under VSI No. G1OA007289 dated October 06, 2023 until such time that it is registered
with the LTO and have subsequently agreed to the warning without any condition, and that any violation of any law pertinent to the vehicle or its use, or damages to it of whatsoever
nature, subsequent from the time of its release, including Fines and Penalties to Dealers and/or Manufacturers for Releasing Unregistered Vehicles, shall be solely for my own account
and liability. Further, that I shall be solely liable to the Dealer for the Cancellation of its Certificate of Accreditation by the LTO and for its blacklisting and cancellation of CSR, sales
report and registration by the LTO.

FINES AND PENALTIES TO CUSTOMERS

1.1 Driving an Unregistered Vehicle

a. Penalty to the Owner depending on duration of no registration
i. One (1) month or less – PHP 10,000.00
ii. More than (1) month – PHP 10,000.00 and impounding of vehicle
b. Penalty to the Driver – PHP 1,000.00
1.2 Failure to Attach Plate
a. If no temporary plate is attached – PHP5,000.00
FINES AND PENALTIES TO DEALERS/ MANUFACTURERS FOR RELEASING UNREGISTERED VEHICLES

PENALTY (to be charged to me, the customer, by reason of this Absolute Waiver)
a. 1st Offense – PHP 100,000.00
b. 2nd Offense – PHP500,000.00 and suspension of not more than six months
c. 3rd Offense – Cancellation of Certificate of Accreditation; and blacklisting and cancellation of CSR, sales report and registration

I further acknowledge that I have read and understood this document in full and unconditionally accept my liabilities arising from, and the consequences of this Absolute Waiver.

I execute this Absolute Waiver which also serves as my quitclaim with the Dealer in connection with the aforesaid vehicle voluntarily and of my own free will on ______________ in
