AFFIDAVIT
I, RUBIELYN T. GARCIA, of legal age, Filipino, Single, and with residence address
at 534 Km.3 Asin Road, Baguio City, after being duly sworn to in accordance with law
do hereby depose and state the following to wit that:

1) I, Rubielyn T. Garcia, the Chairman of Precinct no. 45, Barangay San Luis at San
Luis Elementary School in Baguio City with the other members of the EB obtained the
paraphernalia on October 29, 2023 from the City Hall and we prepared the polling
precinct and other materials that will be needed for the October 30 Election.

2) In the morning, we managed to get things run smoothly. However by 11 AM I

started to get dizzy and nauseous but I can still perform my duty as a chairman.
However, by 12 AM, my condition isn’t getting better. I was telling my co EBs. I
reported the incident to our DESO staff. By 1:30, I asked him if I could be substituted
since we are only halfway done and I wasn’t getting any better. The latter asked me to
relax and asked if I could go home alone. I told him that I was waiting for my fetcher.
The personnel of COMELEC went to the precinct to check on me but I was already
home. They inspected the place and said that it really lacks ventilation and very small.
The EB members were seated together and the voters were just 1 or two inches away.

3) I rested for 2 hours in the house. By, 4 PM, Maam Drealin of COMELEC BAGUIO
called me asked me if I was ok. I was a little better but felt still very weak. She encouraged me to
go for a medical check-up. My mother accompanied me to BGH for a check-up. The doctor said
that the dizziness and nausea was caused by Vertigo (BVVP) and could have been triggered by
stress and fatigue. It was also triggered by the place since our precinct only has the door as the
source of ventilation She prescribed a two-week medicine and recommended me to rest. Now, I
am recovering from it.

4) That I am executing this Affidavit to attest to the truth of the foregoing

facts and circumstances for all legal intents and purposes it may serve.
