1.0 Purpose/ Scope

It is the Java PU policy to maintain a safe workplace for all employees. This
document provides the minimum competency requirements and specifications for
the safe operation of cranes, hoisting and rigging equipment. It also describes
the personnel qualifications and training necessary for the safe operation of
cranes, hoists and rigging equipment.

The safety rules and guidance in this document apply to all work performed in the
Java Performance Unit (JPU) that involve the use of cranes, hoists and rigging
equipment. It applies to all employees, including contractors and subcontractors,
and equipment working in the JPU. Contractors and subcontractors may use
their own program or comply with this standard as a minimum operational
requirement, whichever is more stringent.

1.1 Competency in Lifting Operations- BP’s Golden Rules of Safety

Our workforce must be competent to perform lifts in compliance of BP’s Golden


Rules of Safety. The Lifting Operations Golden Rule expectations are stated
below:

“Lifts utilizing cranes, hoists, or other mechanical lifting devices will not
commence unless:
§ An assessment of the lift has been completed and the lift method and
equipment has been determined by a competent person(s)
§ Operators of powered lifting devices are trained and certified for that
equipment
§ Rigging of the load is carried out by a competent person(s)
§ All lifting devices and equipment have been visually examined before each
lift by a competent person(s)”

A “Competent” person is defined by the Occupational Safety and Health Act as:

"One who is capable of identifying existing and predictable hazards in the


surroundings, or working conditions which are unsanitary, hazardous, or
dangerous to employees, and who has authorization to take prompt corrective
measures to eliminate them.”

BP’s “Competencies On-Line” (COL) describes competency as:

“The things people need to be good at if they are to be effective in their jobs and
meet the needs of the organization. They are a combination of knowledge, skills,
behaviors and aptitudes which produces good performance.”

Competent Person Definition: "One who is capable, by acquired knowledge,


demonstrated skills and experience, of identifying existing and predictable

hazards in the surroundings, working conditions, or behaviors, which are
hazardous, or dangerous to employees, and who has authorization to take
prompt corrective measures to eliminate them.”

Within this context, there are broad roles personnel fill when involved in “Lifting
Operations” and require specific competency-based training and certification:
(examples, but not a total list) Offshore Installation Managers, Projects Manager
and Drilling & Completion Managers will be accountable for deciding who will be
trained to these roles and responsibilities.

1. Rigger-Signalman – mechanic, roughneck, swapper, roustabout, pipeline


operators, delivery vendors, boat crew etc.
2. Crane-Hoist Operator – truck driver, mechanic, drillers, service unit
operators, etc.
3. Lift Leader – TLs, Operations Specs. Tool Pushers, Field Supervisors,
Field Techs, Plant Operator.
4. Forklift Operator.

All personnel involved in mechanical handling and lifting operations on BP


worksites shall be trained and certified to recognized industry standards, have
the requisite experience and be formally assessed as competent in the tasks to
be undertaken. Training and certification for personnel involved in lifting
operations shall be to industry recognized standards.

Documentary evidence of competency will be required before any person is


authorized to perform lifting operations. Ongoing training and certification
to assure competency of all personnel authorized to perform lifting
operations shall be the responsibility of the personnel’s employer

3.0 Personnel Qualifications Requirements and Objectives

This section defines the personnel qualification and training necessary for the
operation of cranes and hoists, the inspection of such equipment and the
performance of rigging operations.

3.1. General
Personnel who are designated to operate lifting equipment to perform work
covered by current contract shall be qualified and trained to the level of
proficiency consistent with their assigned tasks.

3.2. Qualifications

3.2.1. General
Only qualified personnel, or trainees under the direct supervision of qualified
personnel, who meet the following requirements, shall be allowed to perform
rigging, operate, or inspect cranes or hoists. “Trainees under the direct
supervision” means that the trainee is within sight of, or direct communications
with, the supervisor and the supervisor is within 50 feet of the trainee.

3.2.2. Qualifications for Crane and Hoist Operators


A. Qualifications for Offshore Pedestal Crane Operator (BPI-COP-3)

To be a qualified offshore Pedestal Crane Operator, the employee shall:

1. Posses valid MIGAS/MENAKER Lifting Equipment Operator Certificate for


the crane he/she operates (Class-A for up to 20 tons; Class-B for 20 tons up
to 50 tons; Class-C from 50 tons up to 150 tons) or OPITO-3 or equivalent
2. Be designated as a qualified operator by Contractor’s Lifting Authority to
operate respective cranes or hoists. This designation is based on having
experience, training (classroom and field training on cranes and hoists
3. Satisfactorily pass Medical Check Up from Medical Centers approved by BP
Indonesia where confirmation of having a good vision is collected.
4. Meet the requirements of a qualified rigger and signalman
5. Complete a formal operator-training program.
6. Receive practical training from a qualified crane and hoist operator or trainer.
7. Satisfactorily pass a written examination covering:
• Crane and hoist operational characteristics
• Load Controls and emergency control skills.
• Crane component
• Understanding on Center of Gravity and characteristic of the load on
dynamics
• Hand signaling
• Rigging technique
• Weather Hazard
• Limit switches and warning devices

8. Complete a practical operating skill evaluation test (actual or simulated),


demonstrating proficiency and basic knowledge in handling the specific type
of equipment for which the operator is being evaluated, including:
• Pre-start and post-start inspection.
• Maneuvering skills.
• Normal & Emergency Shutdown
• Securing the crane or hoist.
• Stabilize the swinging hook block
• Hand signaling or communication technique
• Boat tangling response
• Boat drifting response

9. Demonstrate the ability to read and load/capacity chart usage, in the


language of the crane manufacturer's operation and maintenance instruction
materials.

Everyone who designated as offshore pedestal crane operator shall be put into
BPWJ Crane Operator Assessment Program to verify his/her competency based
on above standard. A BPI-COP-3 license will be awarded to indicate a
successful completion of the program and passed the minimum score.

B. Qualifications for Offshore Pedestal MAN-RIDING Crane Operators (BPI-


COP-3P)
To be a qualified offshore MAN RIDING crane operator, the employee shall
holding level competency of BPI-COP-3 as stated above plus:

1. Able to describe Personnel Transfer Capsule (FROG)


2. Demonstrate the ability to visual inspect critical parts and operate the
FROG
3. Understand operational limitation of the FROG
4. Understand operational procedure of FROG
5. Completed Personnel Transfer Carrier training

Everyone who designated as offshore pedestal MAN RIDING crane operator


shall be put into BPWJ Crane Operator Assessment Program to verify his/her
competency based on above standard. A BPI-COP-3P license will be awarded to
indicate a successful completion of the program and passed the minimum score.

C. Qualifications for Onshore Mobile Crane Operators (BPI-COM-2)
This section applies to crawler cranes, the wheel mounted cranes of both
truck and ype, and any variations thereof, which retain the same fundamental
characteristic. This type of crane is basically powered by internal combustion
engines or electric motors and utilizes drums and ropes for hoisting.

To be a qualified Onshore Mobile Crane operator, the employee shall:

1. Posses valid MIGAS/MENAKER Lifting Equipment Operator Certificate


for the crane he/she operates (Class-A for up to 20 tons; Class-B for 20
tons up to 50 tons; Class-C from 50 tons up to 150 tons) or OPITO-3 or
equivalent
2. Be designated as a qualified operator by Contractor’s Lifting Authority to
operate respective mobile cranes . This designation is based on having
experience, training (classroom and field training on cranes)
3. Satisfactorily pass Medical Check Up from Medical Centers approved by
BP Indonesia where confirmation of having a good vision is collected.
4. Meet the requirements of a qualified rigger and signalman
5. Complete a formal operator-training program.
6. Receive practical training from a qualified crane trainer.
7. Satisfactorily pass a written examination covering:
• Mobile crane and hoist operational characteristics
• Load Controls and emergency control skills.
• Mobile crane component and structure
• Understanding on Center of Gravity and characteristic of the load on
dynamics
• Hand signaling
• Rigging technique
• Weather/Power line Hazard
• Limit switches and warning devices
• Basic Hydraulic system
• Load Chart Static and Dynamic reading and implementation
• Lifting Operation Planning
• Risk Assessment
• Understanding on Lifting Team role & responsibility
• Lifting Operations Rules
• Mobile crane pre use inspection

8. Complete a practical operating skill evaluation test (actual or simulated),


demonstrating proficiency and basic knowledge in handling the specific
type of equipment for which the operator is being evaluated, including:
• Pre-start and post-start inspection.
• Maneuvering skills.
• Normal & Emergency Shutdown
• Securing the mobile crane or hoist.
• Stabilize the swinging hook block
• Hand signaling or communication technique
• Safely parked the crane

9. Demonstrate the ability to read, comprehend and exhibit arithmetic skills
and load/capacity chart usage, in the language of the crane
manufacturer's operation and maintenance instruction materials.

Everyone who designated as onshore mobile crane operator shall be put into
BPWJ Crane Operator Assessment Program to verify his/her competency based
on above standard. A BPI-COM-2 license will be awarded to indicate a
successful completion of the program and passed the minimum score.

D. Qualifications for Offshore Mobile Crane Operators (BPI-COM-3)


To be a qualified offshore mobile crane operator, the employee shall holding
level competency of BPI-COM-2 as stated above plus on site practical test
on:

1. How to parked the crane safely


2. Load swinging control or mitigation
3. Understanding marine stability (barge vs crane position vs sea state)
4. Safety rules of congested area crane operations

Everyone who designated as offshore mobilel crane operator shall be put into
BPWJ Crane Operator Assessment Program to verify his/her competency based
on above standard. A BPI-COM-3 license will be awarded to indicate a
successful completion of the program and passed the minimum score.

E. Qualifications for Overhead Crane Operators (BPI-COH)


Qualified Overhead Head Crane Operator shall have minimum the following
requirement:

1. Good knowledge on Beam, Hoist capacity.


2. Adequate experience on operating various type of overhead crane such as
but not limited to: Pneumatic Hoist, Electrical Hoist, Hydraulic Hoist.
3. Good knowledge in electrical safety device such as breaker, switches etc.
4. Capable for assignment as Qualified Overhead Crane Operator by
Contractor’s Lifting Authority to operate respective cranes or hoists. This
designation is based on experience, training (classroom and field training on
cranes and hoists)
5. Satisfactorily pass Medical Check Up from Medical Centers approved by
COMPANY where confirmation of having a good vision is collected.
6. Meet the requirements of a qualified rigger and signalman
7. Have a formal operator-training program.
8. Receive practical training from a qualified overhead crane and hoist operator
or trainer.
9. Satisfactorily pass a written examination covering:
• Overhead hoist operational characteristics
• Load Controls and emergency control skills.
• Overhead Crane component

10. Demonstrate the ability to read and load/capacity chart usage, in the
language of the crane manufacturer's operation and maintenance instruction
materials.

F. Qualification for Offshore Tower Crane Operator (BPI-COT-3)

To be a qualified Overhead Head Crane Operator, the employee shall:

1. Posses valid MIGAS/MENAKER Lifting Equipment Operator Certificate for


the crane he/she operates (Class-A for up to 20 tons; Class-B for 20 tons up
to 50 tons; Class-C from 50 tons up to 150 tons) or OPITO-3 or equivalent
2. Pass qualification as Overhead Crane Operator (BPI-HO-1)
3. Be designated as a qualified Tower Crane Operator by Contractor’s Lifting
Authority to operate respective cranes or hoists. This designation is based
on having experience, training (classroom and field training on cranes and
hoists
4. Satisfactorily pass Medical Check Up from Medical Centers approved by
COMPANY where confirmation of having a good vision is collected.
5. Receive practical training from a qualified crane and hoist operator or
trainer.
6. Satisfactorily pass a written examination covering:
• Crane and hoist operational characteristics
• Load Controls and emergency control skills.
• Tower Crane component
• Understanding on Center of Gravity and characteristic of the load on
dynamics
• Hand signaling
• Rigging technique
• Weather Hazard
• Limit switches and warning devices
• Basic Hydraulic system
• Load Chart Static and Dynamic reading and implementation
• Lifting Operation Planning
• Risk Assessment
• Understanding on Lifting Team role & responsibility

• Crane pre use inspection

7. Complete a practical operating skill evaluation test (actual or simulated),


demonstrating proficiency and basic knowledge in handling the specific type
of equipment for which the operator is being evaluated, including:
• Pre-start and post-start inspection.
• Maneuvering skills.
• Normal & Emergency Shutdown
• Securing the crane or hoist.
• Stabilize the swinging hook block
• Hand signaling or communication technique
• Boat tangling response
• Boat drifting response

8. Demonstrate the ability to read and load/capacity chart usage, in the


language of the crane manufacturer's operation and maintenance instruction
materials.

G. Qualification for Forklift Operator (BPI-FO)

To be a qualified as Forklift Operator, the employee shall have :

1. Formal Forklift training program by recognized training provider at least


register in MIGAS or DEPNAKER or International Training Provider.
2. Be designated as a Forklift Operator by Contractor’s Lifting Authority. This
designation is based on having experience, training (classroom and field
training on rigging activity)
3. Satisfactorily pass a written examination covering forklift operating standard
provide by company

3.2.3 Qualifications for Riggers/ Load Handler (BPI-RIG)


To be qualified as a rigger/load handler, the employee shall:
1. Be designated as a qualified rigger by BPWJ Lifting Technical Authority to
perform rigging activities. This designation is based on having experience
and training (classroom and field training in rigging. Contractors and
subcontractors are responsible for qualifying their own employees.
2. Complete a formal rigging training program, or equivalent.
3. Receive hands-on training from a qualified rigger or trainer designated, or
approved, by BP Indonesia.
4. Satisfactorily pass a written examination covering the use and characteristics
of the various rigging equipment to be used.
5. Pass a practical rigging skill evaluation test (actual or simulated), that
requires the use of rigging equipment in safe configurations and
demonstrating proficiency and basic knowledge in handling the specific type
of equipment for which the operator is being evaluated, including:
• Correct sling angles.
• Maneuvering skills.
• Center of gravity of a load.

• Securing the load.
• Securing the load to the crane.
6. Demonstrate the ability to read, comprehend and exhibit arithmetic skills and
sling capacity chart usage, in the language of the equipment manufacturer's
operation and instruction materials.

Everyone who designated as Lifting Rigger/Load Handler shall be put into BPWJ
Crane Operator Assessment Program to verify his/her competency based on
above standard. A BPI-RIG license will be awarded to indicate a successful
completion of the program and passed the minimum score.

3.2.4. Qualifications for Inspectors (BPI-LIFT-INSP)


Qualified inspectors shall have the necessary knowledge and experience to properly
inspect hoisting and rigging equipment. The knowledge and experience may be
obtained from actual work experience or from formalized instruction.

Persons must be designated by WJPU’s Lifting Technical Authority to inspect cranes,


hoists and rigging equipment. This designation is based on having appropriate
experience, training, being designated as a qualified operator and having successfully
completed formal training on crane inspections, maintenance and troubleshooting, in
hoist troubleshooting and repair, and the structural aspects of cranes which provide the
designee with knowledge of critical components and inspection criteria.

A qualified inspector can perform pre-use, initial, monthly and annual inspections.

Inspections shall be limited to the type of equipment for which the inspector is qualified.

Upon successful completion of training, the qualified inspector and the inspector’s
supervisor will complete and sign the inspector’s qualification certificate.

3.2.5. Qualifications for Signalman/Banksman (BPI-SIG)


These people will be qualified by experience and knowledge of the appropriate hand
signals and crane and rigging operations. These people will generally be qualified
riggers.

Everyone who designated as signalman/banksman shall be put into BPWJ


Signalman/Banksman Assessment Program to verify his/her competency based on
above standard. A BPI-SIG license will be awarded to indicate a successful completion
of the program and passed the minimum score.

To be a qualified as a Signalman, the personnel shall:

1. Formal Rigging training program by recognized training provider at least


register in MIGAS or DEPNAKER or International Training Provider
2. Be designated as a Signalman by Contractor’s Lifting Authority. This
designation is based on having experience, training (classroom and field
training on rigging activity)
3. Posses a valid qualification as Rigger (BPI-RIG).

4. Understand and capable to demonstrate the API-RP2D Hand Signaling
(Appendix-B).
5. Capable to use radio communication and ability to speaks clear
communication.
6. Understand that the appropriate lifting accessory and safe slinging
technique is identified
7. Understand on how to assess all proximity hazards and how to eliminate or
mitigate them.
8. Knowledge on lifting and rigging equipment’s inspection
9. Capability to select proper lifting equipment
10. Capability to choose lifting technique.
11. Capability to Stop lifting operation if any unsafe condition.

3.2.6. Qualifications for Supervisors (BPI-LIFT-SPV)


The supervisors of crane, hoisting and rigging operations shall be knowledgeable of the
specific types of hoisting and rigging operations under their supervision, the operational
hazards of those activities and the inspection requirements for cranes, hoists and rigging
equipment. Supervisors are not required to be qualified operators or riggers.

3.2.7. Qualifications for Maintenance Persons (BPI-LIFT-MAINT)


Complete maintenance of cranes and hoists is beyond the scope of this standard.
Anyone performing complete maintenance of this equipment should be qualified by the
equipment manufacturer to perform that work. Qualified operators are considered
qualified to perform minor maintenance on cranes such as lubrication, minor
adjustments and similar activities. Qualified riggers are considered qualified to perform
minor maintenance on rigging equipment such as lubricating wire rope, cleaning alloy
chain and similar activities.

3.3. TRAINING
The Indonesia’s MIGAS/MENAKER, BP Golden Rules and Occupational Safety
and Health Administration (OSHA) require only trained and designated personnel
operate cranes and hoists.

3.3.1. General
Operations Availability Team shall provide training programs for all BP crane
operators, riggers and inspectors, including a means of evaluation, to ensure that
the personnel are competent to perform the operations. Training programs shall
be based on the needs of the individual. BP’s contractors and subcontractors
shall responsible for completing the training plan and the program stated below
are the minimum training requirement.

Training programs will address three levels of performance:


1. Personnel who operate mobile cranes, floor operated cranes, pendant-
controlled cranes, and manual hoists as part of their normal work
assignment.

2. Personnel who perform rigging as part of their normal work
assignment.
3. Personnel who perform equipment inspections as part of their normal
work assignment.

3.3.2. Crane and Hoist Operators

hoist operator and qualified inspector must be familiar and which must be
covered in the training.

3.3.3. Riggers
The following list contains basic information with which a qualified rigger must be
familiar and which must be covered in the training.

3.4. REQUALIFICATION
Operator, rigger, and signalman/ banksman qualification is for a period not to
exceed 2 years, unless the Technical Authority revokes the qualification sooner.

The program for re-qualification shall include:


1. Satisfying the requirements for qualification,
2. Completion of a written and practice evaluation,
3. Satisfying the requirements for qualification, include minimum
requirement of certification or training as require by Indonesia’s
government regulation.
4. Completion of Verbal and Interview Evaluation.
5. Participating in company refresher training, or
6. Completion of a performance evaluation.

3.5. RECORDS

A record of training and skill evaluations shall be kept on file. Contractors


and subcontractors records shall be made available to BP upon request.
Java Lifting Technical Authority shall maintain the shortlist qualified
operator, rigger, signalman and lift leader.
