Usability Test for Jonathan Hsin’s Instructional Video

Objective: To test the effectiveness of the instructional video for how to create a discord
server.
This is a consent form. Please read the following information thoroughly and sign this
form:

I agree to participate in an online usability test using a computer. During the

usability test, I will be interviewed about my basic information and experience
with technology and asked to find information or complete a specific task using a
student-developed instructional video.

I understand and consent to the use of the results by the VT student researcher. I
understand that the information is for research purposes only and that my name
and image will not be associated with any data collected during this study. I
relinquish rights to any video or audio recordings collected during this session.
I understand that participation is voluntary and that I may stop participating in the
test at any time.

By signing this form, I confirm that I have read and understand the information
provided above. I agree to participate in the study and give my consent for the
researcher to use data collected during the study for their class research
purposes.
