Title:
Employer document

Initiated by: EMPLOYER X CONTRACTOR
Name of initiator:

Name of analyser:
A. Description and reason of change:
Reference:
This Management of Change details a deviation from the Procedure Crew Transfer and refers requirements
as made in the Employer Requirements.
- Procedure Crew Transfer
- SHE Requirements

Description of change:
Vessel is currently performing trenching ops. Contractor maintains a regime of ‘’ block changes ‘’ in which
personnel on the vessels are swapped in groups. In principle this means that no regular (or substantial) crew
changes are foreseen whilst the vessel is performing works offshore. However, a small amount of
unforeseen crew changes may still be necessary in order to progress the offshore operations.

Transfer method (deviation from Procedure Crew Transfer)

As the boatlanding has been removed from the vessel due to technical reasons, a FROG device has been
placed on the vessel to be able to safely carry out these unforeseen crew changes. As per the Employer
Requirements the use of the FROG is only allowed based on the ALARP principle. All other proposed solutions
are either not feasible or provide a substandard level of safety (such as the use of the Pilot ladder). Above
that, going to a quayside/port for these type of transfers is not practicable nor sensible with regards to safety
for both the operation and vessel.

Preparation and approval process

following:

1. This MOC including a JSA is setup detailing the risks and mitigations of the FROG transfer, which
can be applied on all FROG transfer operations to and from the vessel during the project.
2. For each transfer operation, permission is requested from Client Project Management and HSE by
means of E-Mail.
