AFFIDAVIT OF EXPLANATION

I, KATHLEEN JOY ROLDAN, of legal age, single, Filipino citizen, and a resi-
dent of Blk22, Lot 23, Hibiscus Street Primavera Homes, PH2, Barangay Darasa,
Tanauan City, Batangas, Philippines, under oath, hereby depose and state that:

1. I was born Yuki-shi, Ibaraki Ken, Japan on 13 April 2004;

2.On 29th of June 2006 me and my mother Gennalyn Roldan Namba trav-
elled from Japan to the Philippines through an aircraft;

3.In the above mentioned travel, the Japan Authority issued me a one page
travel document;

4.The travel document contained the following details:

Name: KATHLEEN JOY ROLDAN
Date: 09 June 2006

5.In 2012, my mother noticed that my travel document was missing, in rela-
tion thereto, an affidavit of loss of the said document is hereby attached as An-
nex “A”;.

6. This year, I submitted my application for Philippine passport, to which the

Department of Foreign Affairs (DFA) asked me to submit an Affidavit of Explana-
tion;

7. In compliance thereto, I hereby execute this affidavit in order to attest to

the truth of the foregoing facts and not for the purpose of fraud, but in connec-
tion with passport application, and for whatever legal purposes it may serve.

IN WITNESS WHEREOF, I hereunto affix my signature this 7th of Novem-

ber 2023 at Tanauan City.

AFFIDAVIT OF LOSS

I, GENNALYN ROLDAN NAMBA, of legal age, married, Filipino citi-

zen, and a resident of Blk22, Lot 23, Hibiscus Street Primavera Homes, PH2,
Barangay Darasa, Tanauan City, Batangas, Philippines,, under oath, hereby
depose and state that:

1.On 29th of June 2006, I, together with my daughter Kathleen Joy

Roldan travelled from Japan going to Philippines through an aircraft;

2. Considering that my daughter was still a minor, Japan Authority is-

sued a one page travel document allowing my daughter to travel with me
outside Japan;

3. Sometime in 2012, I noticed that the travel document of my daugh-

ter is missing;

4.Despite diligent efforts, I have failed to locate the said missing docu-
ment up to the present and I now consider it irretrievably lost;

5.I am executing this affidavit to attest to the truth of all the foregoing
and in order to support my and/or my daughter’s application to replace the
lost one, application for Philippines passport, and for any other legal pur-
poses.
