Brief History of Patient:

From Birth Osteogenesis Imperfecta

According to father his child suffering from chest probrom and admitted many
time for illness.
15-03-23 Admitted in private hospital for complaint of bronchopneumonia.
On 01-04-23 the patient developed High-grade fever and rash.

On 03-04-23: patient’s condition couldn’t got better so taken to SIDH Hospital and was admitted
as a suspected case of Measles. Surveillance Officer Dr. Naseem visited hospital and
investigated the case on same day. Blood sample collected and sent to lab on 06-04-23. She
passed away on 05-04-2023 at 11:40 Pm.

Risk factors:

 There is no travel history.

CASE RESPONSE ACTIVITY:

On the directives of District Health Officer Dr. Shakeel Qureshi and Dr. Shams jokhiyo ADHO
(A&A) & Preventive case response activity was conducted in coordination with Dr. Awais khatti
(TSC) and Kishwar Jehandad DSO-EPI. The case response activity was conducted by Dr.
Kishwar and Dr Nudrat EPI focal person UC-10 Phelwan Goth. A teams was sent to the area for
mop-up activity 14 MR doses were given to children according to Measles Guide lines Pakistan.
Monitored by Dr Awais and DSO kishwar jehandad.

Clustering Activity:
No any suspected measles case found during active case search. Active case search was
conducted on 07-04-23 by Dr. Kishwar and Dr. Nudrat and Monitored by DSO EPI East and-TSC-
Gulshan e Iqbal town.

Lab result Status:

Case became positive on 11-04-23
