The Alerts are provided to all Drilling and Workover Operations Foremen and Liaisonmen so that we all learn from the
experiences of others. The knowledge of this incident could prevent similar ones from happening in the future.
Saudi Aramco policy requires the information in this Alert to be communicated to all crew members and documented on
morning reports.

What happened?
A Service company crew consist of Driver, Helper and
Inspector started the journey on their pick-up at 6:40
A.M. from old location that is approximately 15 km
away from new location to conduct inspection. While
crossing Haradh - Yabreen Highway (single blacktop),
they collided with another truck at 7:01 A.M. resulted
in driver fatality on spot and helper passed away on the
way to the hospital while inspector has been
hospitalized at king Fahad hospital on Hofuf.

Consequences:
➢ Two Fatalities and one Lost Time Injury
➢ Property damage

Why did it happen?


Incident is still under investigation, these are preliminary findings:
➢ Service company driver crossed the high-way suddenly and cut the road in
front of the truck, (100% liability).

What should be done to prevent recurrence?


➢ Ensure all drivers enrolled & refreshed in Defensive driving training.
➢ Ensure that road is safe prior to crossing.
➢ Drivers shall understand and follow traffic rules.
➢ Drivers to be vigilant and focused on driving and avoid being distracted.
➢ Drivers must drive defensively while using road.

Areas Where Improvement is Needed:

Lifesaving Rule No. 5 – Road Safety

• Promote driver’s values, attitude, and knowledge on traffic rules and


regulations.
• Defensive driving strategies provide drivers with time needed to react to the
unexpected.
• Two seconds rules shall be followed and extend the distance between
vehicles if needed.
