ORIGINAL PRESCRIPTION WITH COMPLETE NAME, SIGNATURE AND
LICENSE NUMBER OF ATTENDING PHYSICIAN
LABORATORY/DIAGNOSTIC REQUESTS WITH COMPLETE NAME,
SIGNATURE AND LICENSE NUMBER OF ATTENDING PHYSICIAN
HOSPITAL BILL
