BASKETBALL PLAYER WAIVER AND CONSENT FORM

1. The undersigned agrees that player voluntarily participates in playing basketball and assumes
all risk associated with such play.
2. The undersigned acknowledges that there are risks and hazards in playing contact sports
such as the risk of possible contact with COVID-19 virus
3. The undersigned acknowledges that there are risk and hazards in playing basketball
including, but not limited to, those caused by court conditions, playing conditions, shooting, and
dunking a basketball, running, jumping, stretching, sliding, diving and colition with other players,
spectators, or stationary objects, any of which may cause death or serious injury to the player.
4. The undersigned acknowledges that the Sangguniang Barangay and Sangguniang Kabataan
g Prinza and all affiliated entities, including but not limited to host sites, officers, referees and
committees are free from any and all liability claims for injury, illness or death by the participants
while playing, practicing and travelling to and from that may incurred during the event
