Page 2, write APPENDIX A as a title, centered, bold, 12 points, Arial. This page should contain also the first part of the instrument known as Part I. Letter to the Respondents / Participants. In here, write a one-page letter to your respondents/participants. Tell them about the title, objectives, and the reason why you want to conduct the research. Do not forget to include ethical concerns. This letter should have your e-signature above your name being the researcher or principal investigator. 2. On Page 3, write the Part II. Socio-Economic Attributes of Respondents / Participants. In here, you are required to extract information from your respondents by asking at least ten (10) characteristics (variables) about him/her. It is expected that open- ended and close-ended questions or a combination of both are asked or present on this instrument. The first item must be about informed consent. 3. On Page 4 (or Starting in Page 4), write Part III. Main Instrument. In here, all questions about each objective are asked. Like in Part II, questions should be a combination of both. In here, there is no specific length. The length is dependent on the main problem (title) and number of objectives. It is expected that your main instrument has at least three (3) objectives. 4. Transform your Hard Instrument (Questionnaire on Paper) into an electronic form using Google. All Parts (I to III) must be on this electronic form. 7. Name your form as Math 15 <APILYEDO_NGALAN> Instrument (e.g., Math 15 CRUZ_JOHN Instrument). 8. Start recreating Part I, Part II and Part III 9. Finally, using the data you have, use all learnings and apply these learnings to your data by analyzing, measuring/estimating (computing), presenting (tables and charts) and describing / interpreting the data. Write on Page 2, RESULTS AND DISCUSSION, center, bold, ALLCAPS, 12 points, Arial, bold 23.
