The Guidance and Counseling Office of LSEI would like to conduct a survey from Grade 12 students
regarding Career Orientation and College Entrance Examination designed to collect insights about their
career choices as they transition to their college period. Below are the procedures in disseminating and
conducting the survey:
1. The Guidance and Counseling office would send the survey to the Grade 12 Level Coordinator.
2. The Grade 12 Level Coordinator will relay and inform the survey to the Grade 12 Advisers.
3. The Grade 12 Advisers will be sending the survey to all Grade 12 students.
3.1 The survey will be sent on December 4, 2023 (Monday) at exactly 8:00 AM during their first
subject in Flipped Classroom.
3.2 The survey will close by the same date (December 4, 2023) at exactly 5:00 PM.
3.3 All Grade 12 students who are able to finish and submit the survey would have one graded
assignment in Practical Research 2 and attendance to all subjects in their flipped classroom.
3.4 For students who will not be able to submit a survey on the deadline would not have a graded
assignment in Practicel Research 2 and will be mark as absent in Flipped Class and In-Class
for one day.
4. All collected information will be treated with utmost confidentiality and will serve as a basis for
the Career Orientation Program of Grade 12 students.
