Wechsler Individual Achievement Test, Fourth Edition: Date

STUDENTX’s academic achievement was assessed using the Wechsler Individual Achievement Test, Fourth Edition
(WIAT-4). This assessment was conducted by PRACTITIONER NAME on DATE. The WIAT-4 is an individually
administered clinical instrument designed to measure the academic achievement of examinees ages 4 through 50,

comprehension items require examinees to read a sentence and then answer a literal question about it. To measure
passage comprehension, examinees read narrative and expository passages and answer literal and inferential
comprehension questions asked by the examiner. Examinees can refer to the passage as needed to answer the
