my wife ie., Kare Gayathri. Due to mistake and oversight and lack of knowledge Ihave not
informed my son's date of birth to the concerned authorities of Amadaguru Mandal,

educational records. Iann entitled to get date of birth certificate of my son from concerned
officials of AmadaguruMandal. Hence, I am making this declaration for grant of Date of Birth
