CORRECTION OF NAME

I, Adekojo Ayishatu Atoke, adult, Female, Trader, Muslim, citizen of the Federal
Republic of Nigeria, of Isale Agbasi Compound , Igboora , Ibarapa Central Local
Government Area in Oyo State, do hereby make oath and declare as follows:
1. That I am the declarant herein.
2. That during bvn registration, my name was wrongly written as Adeleke
Ayishatu Atoke.
3. That on my NIN slip my name was written as Adekojo Ayishatu Atoke.
4. That this affidavit is now needed to confirm that my correct name is Adekojo
Ayishatu Atoke as it appears on my NIN slip.
5. That this affidavit is now needed for official/record purposes and authority
concerned should please take note .

That I swear to this affidavit in good faith believing same to be true and correct in
accordance with the Statutory Declarations Law, Cap 158, Volume V, Laws of Oyo
State of Nigeria, 2000.
