FORENSIC CERTIFICATION PROGRAM

CERTIFICATION ASSESSMENT TOOL

General Instruction:

The certification proceedings shall be made after all the academic requirements
and the practical exercises of the modules have been met by the trainee. This assessment
tool shall be used by the assessor/mentor-in-charge to determine skill and competency
acquirement of the trainee, as final completion for the program. Such tool serves as a
minimum guide for the assessor/mentor-in-charge. Additional questions can be made as
a prerogative by the assessor/mentor-in-charge.
