PARENTAL CONSENT AND LIABILITY WAIVER

Event: Diocesan Altar Server's Day

I, [Parent/Guardian's Name], am the legal parent/guardian of [Child's Full Name], hereinafter referred to
as the "Participant," who is an altar server in the Diocese of Malolos.
In consideration of my child's participation in the Diocesan Altar Server's Day event on October 21, 2023,
organized by the Diocesan Ministry of Altar Servers Executive Committee, I hereby agree to the
following terms and conditions:
1. I give my permission for my child to participate in the Diocesan Altar Server's Day event and promise
to hold the Diocese of Malolos, the Diocesan Ministry of Altar Servers, their officers, agents, employees,
volunteers, and the event organizers harmless from all liability resulting from or connected with my
child's participation in the event.
2. I am aware that although while the organizers will take reasonable measures to safeguard the security
and wellbeing of every participant, I admit that mishaps or events are nevertheless possible. I
acknowledge that I will bear any risks related to my child's participation.
3. I provide permission for the event's organizers and their representatives to get my child the appropriate
medical care in the event of an emergency, but I agree to be financially responsible for any costs incurred.
4. I give the event's organizers permission to freely utilize any images, recordings, or other likenesses of
my child that were captured during the event for marketing or instructional reasons.
By signing this waiver, I affirm that I have read and understood its contents and that I voluntarily agree to
its terms. I acknowledge that this waiver is binding and shall apply to all activities related to the Diocesan
Altar Server's Day event.
