SWORN STATEMENT

with law, do hereby depose and state that:

1. I am ______________________ of STI College Tagum Inc., located at Apokon Road,

Tagum City, Davao del Norte, a duly registered corporation with SEC and permitted
to operate Senior High School Level with DepEd under Government Permit R-XI No.
125, s. 2021.

2. As Senior High School Faculty member, I sworn my willingness to serve STI College
Tagum Inc. for the School Year 2023-2024 and follow the rules and regulation of the
school in accordance to the contract.

3. I am executing this sworn statement to attest to the truth of all the foregoing
statements and for whatever legal purposes it may serve.
