Pre-Employment Requirement and Conditional Employment

Dear Teammate,

Due to logistical challenges, the pre-employment medical examination (PEME) and the
background check will have to be accomplished during your employment period.

Thus, the background check will be accomplished by the Company’s third-party vendor
during your employment period. Also, you must accomplish your PEME within thirty (30)
days from the start of your employment period.

The failure to satisfactorily accomplish the PEME within the period provided and/or any
adverse finding in the background check regarding any false declarations may lead to
the rescission of your Employment Agreement and the termination of your employment
with the Company.

Thus, your employment is subject to the condition of your satisfactory accomplishment

of your PEME and your truthfulness in your declaration of facts in your application.
