Dear Mr. Janmig,

We have received your request for your Leave of Absence from October 27, 2023 to November 12, 2023 for the
reason that you will be busy with election campaign and your presence in your Barangay is crucial.

With this, we are pleased to inform you that a Leave of Absence is granted under the following conditions:

1. Your salary for for these dates will be put on hold due to your LOA. This is unpaid given that you have
already consumed all your earned leave credits.

2. Failure to return/report to work upon agreed to date will be interpreted as Unauthorized Leave and
corresponding penalty up to termination shall be imposed. Your agreed return date is on
(November 13, 2023). You also agree to call your Team Leader prior to expected reporting date to
confirm work schedule.

3. Upon return to work, you will need to present all supporting documents validating this leave. Your
failure to comply with this would invalidate the reason you’ve previously given and could render your
absence unauthorized.

4. This was evaluated based on the merits of this specific case (performance & circumstance) and does
not serve as a precedent for future requests.

We would like to remind you that other conditions not stated above should have prior approval from the
Operations Management Team. Otherwise, this will be considered as absence without official leave or
unauthorized absences and appropriate action shall be imposed accordingly.

Please indicate your acceptance by affixing your signature that this has been discussed with you and that you
accept all the conditions mentioned.
