The following must accompany this 4-page application form:

● A transcript of your Grade 11 final marks and an official document from your school showing your Grade 12
marks to date (e.g., letter from school, report card, etc.)
● A letter of reference from one of your JDFFC coaches
● A letter of reference from one of your teachers

2023 Scholarship:
All applications and supporting materials including reference letters must be received by JDFFC no later than May 31st,
2023, to be considered by the Committee. All material should be emailed to scholarship@jdfsoccer.ca. Letters of
reference, transcripts and official documents can be sent directly to scholarship@jdfsoccer.ca. It is the candidate’s
responsibility to ensure their application is complete and that JDFFC receives all items as noted in Application
Requirements above by the May 31st, 2023 deadline.
