AFFIDAVIT

I,ALEX NDIVO ANISA OF ID NO 34075761 a resident of Kitui County in the Republic of

Kenya, do hereby make oath and state as follows:-

1. THAT I am an adult male Kenyan of sound mind hence competent to swear this
affidavit.

2. THAT in June,2020 I married one KAVESA KISILU of ID No 37437557 under

Kamba Customary laws and traditions, and have since then cohabited as husband and
wife .

3. THAT we are for all intents and purposes husband and wife and are recognised as such
by all who know us.

4. That I intend to include my wife , KAVESA KISILU as a beneficiary under my NHIF

Cover by virtue of being my wife .

5. THAT I swear this affidavit in declaration that I am married to KAVESA KISILU and
that I intend to include my wife one KAVESA KISILU as a beneficiary under my NHIF
cover.

6. THAT what is deponed herein is true to the best of my knowledge, information and
belief.

confirm on oath as follows:
1. THAT I am a female adult Kenyan of sound mind.

2. THAT I confirm the averments above, and in particular, that we got married in
June,2020 under Kamba Customary laws and traditions and that we are for all intents
and purposes husband and wife and are recognised as such by all who know us

3. THAT I aver the above matters truthfully.
