AFFIDAVIT

I, EUGENE A. ZULUETA, of legal age, Filipino citizen, married to MYLINE F.

ZULUETA, and a resident of Chico Street, Golden Mapandan, Pangasinan, after having been
duly sworn to in accordance with law, do hereby depose and state;

1. That we are the owners of Lot 3 (TCT No. 026-2021003625) and Lot 4 (TCT No.
026-2021003624), (PSU-01-001793) located at Brgy. Loyan, Mapandan, Pangasinan,
with an area of Fourteen Thousand Nine Hundred Thirty-Four (14,934) square
meters, more or less.

2. We hereby manifest and guarantee before this Board that in conducting in sales
transaction with the buying public it shall include/incorporate in its Contract to Sell
from Section 17, 18, 23, 24, 25, 26 and 30 of P.D. 957 (or BP 220) and Section 24, 30
and 31 of its Implementing Standards, Rules and Regulations: (for
subdivision/townhouse/condominium project) Section 17, 18, 19, 22, 23 and 25 of the
New Rules and Regulations Implementing Subdivision.

3. That we are executing this Affidavit to attest to the veracity of the above mentioned
facts thereby satisfying the requirements of the Department of Human Settlements
and Urban Development (DHSUD) relative to my application for registration and
License to Sell for the above-mentioned project.
