Subject: Tardiness (1st offense)

Dear Sir/Madam,

The purpose of this letter is to apologize for my tardiness last April 13, 2023 and April 19, 2023. I was
late for 18 Minutes last April 13, 2023 because of unexpected road widening on our subdivision and I did not
expect that the usual time of my departure will be delayed by the road widening.

I was late for 66 Minutes last April 19, 2021 because of incident happen on our home. To describe fully
the incident, at 9:32 PM in the evening at the Advincula’s residence. I was about to open our gate when I heard
a loud hammering noise near our water meter, little did I know a bunch of teenager are trying to steal our meter
based which is made of bronze, accidentally they hit the water pipe. As the only adult during that time I cannot
leave my 17 Years Old sister unattended. I successfully drive them away by opening our garage light and then I
called my uncle who lives in the neighborhood and ask for his assistance about the broken pipe and I called my
immediate family to tell them the said incident. Luckily no one is harm and the broken pipe was fixed. I was
planning not to go to the Hospital that Night since My sister was all alone, that’s when the time a Senior
medtech staff tried to reach me out and asked if I will be absent that night, and I realize that there are only two
of them will duty that night without me. Therefore to secure my Sister’s safety, I decided to go to hospital once
my mom got home.

To make up for my tardiness. I will render two hours of work today before my regular shift and do the
necessary task within that period. I sincerely apologize for my tardiness. As a Medical Technologist staff, I
know the higher ups might reprimand me for my mistakes. Hence please accept my apologies. Since it was the
first time as an employee, I was involved in such an incident. I have received a verbal and written warning
about these matter. To prevent this incident from happening again, rest assured that this will be the last time I
will be late without immediately informing you.
