3. Authenticated copy of certificate of eligibility/ rating & license (PRC);
4. TIN, BIR Certification, PHILHEALTH ID, MDR, PAG IBIG ID and MDF;
5. Police and NBI Clearance; and
6. Position Description Form (if applicable)
