Dear Felicity, we are pleased to offer you full-time employment in the


position of Personal caregiver with Green-hill aged care (Green-hill
aged care).

The purpose of this letter is to outline the terms and conditions of


employment with Green-hill aged care. You will also find attached a
copy of the Fair Work Information Statement which we have provided
to you as required by law. If you have any feedback or would like to
discuss anything, please contact me. To confirm your acceptance of
this offer, please sign and date this letter at the end of this document.

Award This contract of employment should be read in conjunction with any


Modern Award that is deemed to be applicable to your employment.
Ordinary hours On average 38 hours per week plus any reasonable additional hours
of work that are necessary to fulfil your duties, or as otherwise required. Work
hours: You will generally be working during our ordinary business
hours, 9:00 AM - 5:30 PM Mon - Fri, inclusive of unpaid meal breaks.
Public holidays applicable to your employment are those in NSW. You
will generally not be required to work during public holidays, but we
may reasonably request that you do so from time to time. You may
refuse to work on a public holiday if our request is unreasonable or
your refusal is reasonable in the circumstances.

Position Personal caregiver

Duties The duties for this position are as follows: Provide personal care to our
clients including assistance with showering, dressing, grooming,
toileting and mobility. Provide incidental transport to appointments,
shopping and social outings. You will be required to perform these
duties, and any other duties which may be reasonably assigned to you,
taking into account your skills and experience. Your duties, position,
levels of responsibility and reporting lines may change during the term
of this agreement and we reserve the right to transfer you to another
position for which you are suitably qualified.

Principal state Your principal state of work during your employment will be NSW.
of work We may require you to work at such other locations, including intra-
state, inter-state and overseas, as may be reasonably necessary in the
course of performing your duties.

Reporting You will initially be reporting to: Facility manager or such other
structure person or position as we may advise in writing from time to time.

Remuneration You will be paid a standard hourly rate of $37.00 per hour. The
following conditions have been absorbed into your hourly rate, and
therefore will not apply to your employment, unless otherwise advised
by Green-hill aged care in writing: • Any allowances that may have
otherwise applied to your employment; and • Any applicable annual
leave loading. Additional hours will be paid at the minimum overtime
rates as prescribed by any Award applicable to your employment. As
an alternative to making payment for additional hours, Green-hill aged
care may agree with you in writing to provide you the equivalent time
off in lieu (subject to any provisions of any relevant Award). Motor
vehicle allowance: $2000 per year. You acknowledge that we may set
off your remuneration against any entitlement owing to you under a
modern award, industrial instrument or applicable industrial
legislation, including, but not limited to, entitlements to overtime and
penalty rates, allowances and leave loading. Your pay net of any tax
and necessary deductions will be paid by direct deposit into your
nominated bank account.
