YAP’s main goal of a safety and health program is to prevent work-related injuries, illnesses, and deaths,
as well as the suffering and financial hardship these events can cause for workers, their families, and their
employers.
YAP team is aware that implementing these recommended practices brings other benefits as well. The
renewed or enhanced commitment to safety and health and the cooperative atmosphere between employers

YAP’S NINE STRUCTURED SAFETY & COMPLIANCE PLAN

fix any hazards that could create injure or cause
illness.

3. IMPLEMENT A REPORTING For each project we develop and communicate a


SYSTEM simple procedure for workers to report any injuries,
illnesses, incidents (including near misses/close
calls), hazards, or safety and health concerns
without fear of retaliation. Also includes an option
for reporting hazards or concerns anonymously.

4. PROVIDE TRAINING We train workers on how to identify and control


hazards using different Hazard Identification
Training Tools.

5. CONDUCT INSPECTIONS Regular inspection on the job site with workers


so they can identify any activity, piece of
equipment, or material that concerns them. Use
checklists and other resources to help identify
problems.

1.0 HSSE EXECUTION/Commitment PLAN

The management of YAP Enterprises Inc. is committed to providing logistics


services to his clients in an accident free working environment, for employees and the
public. We promote safe working practices and ensure that all reasonable and
practical action is taken within the legal and regulatory tenets of our operations, to
make this a reality.

This HSSE Execution Plan provides a specific statement of the work plan to organize, perform,
and execute the HSSE Management system of the contracted works under AML responsibility.
It is based on the principles of OHSAS 18001,
YAP Enterprises Inc. Our Values and Guiding Principles, Beyond Zero program and YAP
Enterprises Inc. standard HSSE Plan template.

This plan describes the processes and procedures for ensuring that AML activities, including
engineering, design, construction, and construction management, are undertaken safely and in
line with contractual and regulatory requirements. Additional procedures will be developed and
implemented as required and as the work progresses.

The HSSE Manager or Designee is responsible for preparing, implementing, and updating this
project-specific HSSE Execution Plan as required, with the approval of the Project Manager,
Construction Manager, and project sponsors.

 Prepare, implement and administer Project Health, Safety and Environment(HSE)


Program/Plan for the execution phase;
 Provide Site Contractor Kick off meeting before mobilization and communicate AML
project HSE requirements and expectations;
 Monitor progress and safety of active contractors; and
 Liaison with Liberia Ministry of Labor and Ministry of Public Works as required.

Occupational / Workplace health and safety policy

1.0 Policy brief & purpose

Our Occupational / Workplace Health and Safety Policy help us preserve the best
possible work conditions for our employees. Every employee has a right to feel safe at
work. Our company is committed to follow legal standards and create a hazard-free
workplace.

Our Occupational / Workplace Health and Safety Policy help us preserve the best
possible work conditions for our employees.

1.1 Scope

This Occupational / Workplace Health and Safety Policy applies to all prospective and current
employees of the company as well as volunteers, contractors and consultants.

1.2 Policy elements

There are two aspects to consider when establishing an Occupational / Workplace Health and
Safety Program: Preventative Action and Emergency Management.

1.3 Preventative action

Preventative action is any action we take to avoid injuries or illness related to workplace conditions.

We’ll conduct periodical risk assessments and job hazard analysis to discover what is likely
to harm emplo yees. We’ll establish preventative measures accordingly.

Potential threats and dangerous situations include but are not limited to:

● Performing tasks on heights, scaffolds, ladders and other unsteady structures


● Chemical substances (toxic, flammable etc.)
● Operating dangerous equipment
● Slippery or uneven surfaces
● Electrical infrastructure
● Noise/temperature
● Quality of air

We’ll take the following preventative measures:

● When employees work in dangerous contexts or locations, we’ll make sure there are
safety precautions like safety nets and ropes.
● We’ll provide protective gear like gloves, protective uniforms, goggles etc.
Using safety equipment is obligatory.
● Inspectors and quality control employees will inspect equipment and infrastructure regularly
● We’ll hold employee training sessions in health & safety standards and procedures.
● All highly dangerous job tasks require at least two employees to be present
● Exposure to chemicals and radiation will not exceed a certain time limit
● Employees who do repairs or cleaning need to put up caution signs
● We’ll prohibit smoking indoors

Also, we’ll enforce a substance abuse policy to protect employees from colleagues’ misconduct.

1.4 Emergency Management

Emergency management refers to our plan to deal with sudden catastrophes like fire, flood,
earthquake or explosion. These depend on human error or natural forces.

Our emergency management involves the following provisions:

● Functional smoke alarms and sprinklers that are regularly inspected by [maintenance supervisor]
● Technicians (external or internal) available to repair leakages, damages and blackouts quickly
● Fire extinguishers and other fire protection equipment that are easily accessible
● An evacuation plan posted on the walls of each floor and online
● Fire escapes and safety exits that are clearly indicated and safe
● Fully-stocked first-aid kits at convenient locations

We’ll also schedule fire drills and emergency evacuations periodically. We will monitor
performance of health and safety procedures and will revise them to ensure higher level of
protection.

1.5 Additional measures

Our company will also keep abreast of changes and try to promote health & safety actively. We will:

● Update our policy according to changes in occupational health and safety legislation.

● Use incentive actions for health & safety (e.g. presenting safe employee awards.)

● Analyze past incidents to discover what went wrong.

Our company will also consult experts or insurance representatives to ensure it complies with
local and international standards.

1.6 Disciplinary Consequences

Every team leader is responsible for implementing this health and safety policy. Employees
should follow health and safety instructions and will be held accountable when they don’t.
We’ll take disciplinary action that may extend to termination when employees consistently
disregard health and safety rules.

2.0 DRUG & ALCOHOL PROGRAM

It is YAP Enterprises Inc. policy that the working environment and project sites should be free
from the influence of alcohol and drugs (Drug and Alcohol Procedure).This will help to ensure
the health and safety of its staffs and others with whom they come into contact, and to maintain
the efficient and effective operations of the project. For these reasons the following rules are
strictly enforced:

 Personnel providing a service on the project work site will not:

 Report or try to report to work when unfit because of the influence of alcohol or drugs
(whether illegal or not) or substance abuse.

 Be in possession of, distribute, or sell alcohol or illegal or un-prescribed controlled


drugs, or misuse legitimate prescription drugs in the workplace.

 Consume alcohol or illegal or un-prescribed drugs or abuse any substance while at


work

 Testing will be completed for Just Cause and Post Incident Testing. Contractors shall
be responsible for all costs associated with testing.

2.1 SIX SAFETY ESSENTIALS

YAP Enterprises Inc. project employees, contractors, vendors and visitors are responsible for
ensuring compliance to the Six Safety Essentials.

The Six Safety Essentials will be introduced to all YAP Enterprises Inc. employees, and YAP
Enterprises Inc. managed contractors, through the YAP Enterprises Inc. site-specific orientation
and Presentations.

2.2 YAP ENTERPRISES INC. SAFETY RULES

YAP Enterprises Inc. managed project employees and contractors are responsible for ensuring
compliance to the 10 YAP Enterprises Inc. Safety Rules. They will be introduced through the
site specific orientations, discussed at the contractor kick off meeting and will be audited for
assurance purposes.

The YAP Enterprises Inc. 10 Safety Rules are applicable to all YAP Enterprises Inc. managed
contractors (Ref. 164596-HSSE-IST-001).
1. Permit to work (required for hot work where applicable, excavations, confined space,
lockout, overhead power lines);

3.0 LEADERSHIP & COMMITMENT

3.1 GOALS & OBJECTIVES

In accordance with the project HSSE strategy, the project will:

3.2 PROJECT HSSE PERFORMANCE TARGETS

YAP Enterprises Inc. and managed contractors shall follow the established leading and lagging
indicators for the project.

The formula for calculation of Lost Time Incident Frequency (LTIF), Total Recordable Incident
Frequency (TRIF), and All Injury Frequency (AIF) is the number of recorded incidents
multiplied by 200,000 and divided by actual man-hours worked.

4.0 LEGAL & OTHER REQUIREMENTS

4.1 LEGAL REQUIREMENTS

YAP Enterprises Inc. contractors and employees will comply with the provisions of all
relevant Legislation, Standards and Best Practices.

During construction phase, audits against applicable legislation will be included and
implemented as part of the assurance strategy for the project.

The regulations applicable to the Road construction project are listed below but not limited to:

Engineering Planning Construction & Maintenance contractual agreements

Liberia, Africa Environmental Management and Protection Act and Regulations

4.2 SOCIO-ECONOMIC REQUIREMENTS

YAP Enterprises Inc. has overall responsibility for socio-economic management. YAP
Enterprises Inc. shall comply with all AML. Socio-economic requirements and support YAP
Enterprises Inc. national related Business and Training initiatives

4.3 ENVIRONMENTAL REQUIREMENTS

YAP Enterprises Inc. has overall responsibility for environmental management on the project
site.

Contractors and employee(s) are required to comply with all their responsibilities under
environmental legislation and conform to YAP Enterprises Inc. environmental management
system/ESIA as it applies to their work on Road construction site. Yes, we do have an
Environmental Protection Manual, but the ESIA and Permit from EPA will suffice above all
other Environmental regulation.

YAP Enterprises Inc. will monitor compliance of its employees and managed contractor(s) to
YAP Enterprises Inc. l environmental requirements.

4.4 SECURITY REQUIREMENTS

YAP Enterprises Inc. has overall responsibility for security management. The purpose of
project security is to protect workers, property, and other assets from injury, harm, and damage
from criminal, hostile, or malicious acts, and by implementing suitable security provisions to
manage identifiable project security needs. YAP Enterprises Inc. will establish site and camp
related security coverage for managed contractors, employees in compliance with YAP
Enterprises Inc. security procedures.

4.5 OCCUPATIONAL HEALTH REQUIREMENTS

Occupational Health will be taken into consideration during all stages of the project. The YAP
Enterprises Inc. HSSE Department will monitor YAP Enterprises Inc. managed contractors’
and subcontractor(s) activities to verify that all are taking the necessary measures to prevent
adverse effects on the health and well-being of YAP Enterprises Inc. managed Iron-ore mining
project personnel.

YAP Enterprises Inc. managed contractor(s) and employee(s) are responsible for ensuring that
their personnel employed on the contract are fit for the work they will undertake.
YAP Enterprises Inc. is responsible for hygiene and welfare arrangements on the YAP
Enterprises Inc. managed Iron-ore mining project site.

YAP Enterprises Inc., is responsible to ensure provision of medical services, qualified


emergency medical personnel and emergency vehicle commensurate to international
requirements and YAP Enterprises Inc. company policy.

Traffic Management Requirements:

YAP Enterprises Inc. recognizes that the operation of workplace transport vehicles is among
the highest risks on construction sites. YAP Enterprises Inc. is committed to the effective
management and elimination or reduction of this risk as part of its overall management of
health and safety.

YAP Enterprises Inc. will implement established rules and procedures to manage project
driving and workplace transport risks on the project, both on and off-site.

All YAP Enterprises Inc. safety requirements for heavy equipment and light duty vehicles as
well as safety arrangements associated with the logistics of moving heavy equipment/ materials
on public/ project roads are provided in the Managing Road Safety.

We will also provide our Vehicle Driving Safety Standard as a minimum requirement.

A description of the key HSSE risks for the project scope of work is provided in the form of a
Risk Register in Appendix 6-1. It is the responsibility of the Project HSSE Manager to facilitate
the population, revision, and maintenance of the Project HSSE Risk Register throughout the
project life.

5.2 RISK ASSESSMENT/ JSA

All work activities on YAP Enterprises Inc. mining site shall be subject to Risk
Assessment.

The YAP Enterprises Inc. risk assessment process comprises two stages:

 Job Safety Analysis (JSA) – Completed on site prior to commencing work,


 Toolbox Talk (TBT) and Task Analysis Safety Card (TASC) – Completed by the team
implementing the job. The TBT and TASC communicate the outcome of the JSA and
consider local hazards that may have changed since the JSA was completed.

YAP Enterprises Inc. Risk Assessment arrangements are documented in SHE-PRO-000013


Risk Assessment procedure.

The YAP Enterprises Inc. Managed Contractors may use the own Risk Assessment procedure
if it is equal to or better than YAP Enterprises Inc. Risk Assessment Procedure. YAP
Enterprises Inc. will review the contractors’ Risk Assessment procedure to ensure YAP
Enterprises Inc. expectations and requirements are met.

As part of Site HSE orientation YAP Enterprises Inc. and its managed contractor’s
supervision will be required to attend basic risk assessment training.

6.0 HSSE IMPLEMENTATION & OPERATION

 Ultimate responsibility for the YAP Enterprises Inc. managed Road Construction
project HSSE Management System and implementation of the Project HSSE Execution
Plan lies with the YAP Enterprises Inc. project manager.
The YAP Enterprises Inc. project construction manager will ensure that HSSE
matters are given due consideration in pre-planning and preparation of work procedures to
allow safe execution and completion of the work, meeting HSSE as well as budget, schedule,
and quality requirements.

The YAP Enterprises Inc. project HSSE manager will be responsible for providing expert
advice and support to project management in matters related to HSSE. The HSSE manager will
also ensure that the project specific HSSE Execution Plan and relevant HSSE procedures are
prepared in accordance with all statutory regulations having jurisdiction over matters relevant to
this project, with input from all relevant parties.

 Actively participate and personally contribute to the safety watch program culture
and expectations;
 Attend when requested, listen, and comply with all instructions provided during
toolbox talks and if in doubt ask for further explanation until satisfied;
 Comply with HSSE working instructions and procedures provided by their supervisor
or foreman;
 Never take shortcuts;
 Fully comply with the permit to work controls and arrangements when required;
 Take advice and comply with any instruction given to them by the HSSE manager or
designate with respect to working in a healthy, safe, and environmentally compliant
manner;

 Not interfere with any tool, item of equipment, or machinery for which they are not duly
authorized, qualified, and/or licensed to operate or maintain;
 Report any unsafe tools or equipment or unsafe methods of work immediately to their
supervisor or foreman;
 Use only the correct tools and equipment for the job and never make or use handmade
tools;
 Ensure good housekeeping is maintained at all times and that waste generated from
activities is suitably segregated and disposed of in the correct skips, bins, or containers;
 Inspect, wear, and look after any safety and personal protective equipment issued to
them;
 Observe all health, safety, and environmental signs and notices and never cross warning
barriers unauthorized;
 Adhere to the “three points of contact rule” when using ladders, stairs, and equipment. Use
the handrail when going up or coming down stairs;
 Immediately report all accidents, incidents, and near-misses in which they have been
involved or have witnessed to their supervisor or foreman, and fully cooperate with any
investigation;
 Take care in preserving the health and safety of themselves and other persons who may
be affected by their acts or omissions at work; and
 Work safely as a condition of their employment.

6.2 HSSE Steering Committee

Meetings of the HSSE steering committee will be held monthly. The meetings will be
attended by YAP Enterprises Inc. project manager, YAP Enterprises Inc. HSE
Representative YAP Enterprises Inc. project manager, YAP Enterprises Inc.
OPERATION manager, and HSSE manager, Contractor Senior Management. Terms of
reference for the meetings are outlined below.

 To monitor the project HSSE statistics with regard to achievement


and potential trends (positive and negative), and to identity
opportunities for improvement;
 To monitor and predict potential HSSE hazards at different stages as
the work progresses, and to provide guidance on action needs at these
times;
 To provide the site JOH&S committee with guidance as required and
with resolutions for HSSE issues and concerns, where required;

 To ensure adequate resources are available to implement agreed-upon


HSSE initiatives and improvements;
 To monitor the achievement of the HSSE Rolling Improvement plan
and provide guidance on the actions needed to correct deficiencies;
 To monitor the ongoing achievement of the project formal compliance
measurement processes and provide guidance on issues of concern;
 To monitor the effectiveness of the HSSE communication processes
and provide guidance on improvement where required;
 To promote leadership and accountability at all levels on the project
and provide feedback on achievement; and
 To take the necessary actions to support cross-project learning in
whatever manner is deemed applicable

6.3 Joint Occupational Health & Safety Committee (JOH&SC)

Project wide Joint Occupational Health and Safety Committee (JOH&SC) will be established in
conformance with Work safely as a condition of their employment. Corporate
requirements. (Ref. SHE-PRO-000021 Joint Occupational Health and Safety Committee). Will
be represented in the project wide JOH&SC.

The committee will be required to hold meetings [section 9(33)] and carry out regular
inspections of the workplace [sections 9(26), 9(27) and 9(28)].

HSSE management personnel from YAP Enterprises Inc. will meet monthly to ensure
alignment and consistency of approach.

Similarly, weekly meetings will be held between YAP Enterprises Inc. and Contractor
HSSE personnel to review project related issues and ensure alignment and consistency of
approach.
6.4 Weekly Contractor HSSE Meetings

Each contractor(s) and subcontractor(s) will conduct weekly HSSE meetings with all his or
her employees to:

 Discuss HSSE performance to date


 Review any HSSE incidents that may have happened during the week
 Discuss any outstanding HSSE issues

 Discuss HSSE issues of upcoming jobs

The contractor and employee(s) will keep minutes and attendance of the meetings; and
The contractor and employee(s) will provide the YAP Enterprises Inc. project
HSSE supervisor with a schedule for weekly HSSE meetings, daily toolbox
meetings and workplace inspections.

YAP Enterprises Inc. project management will arrange to have a YAP Enterprises Inc.
designated individual from the project construction management team attend these meetings.

6.5 Project Contractors Toolbox Talks (TBTs)

The YAP Enterprises Inc. managed contractors shall ensure that project contractor personnel
receive daily job specific and general Toolbox HSSE briefings.

TBTs will be carried out by Contractor Supervisors/Foremen to ensure that hazards relating to
the task are clearly identified and the relevant precautions are explained.

The YAP Enterprises Inc. field coordinator/supervisor/foreman responsible for the


contractor(s) and employee(s) crew will attend the daily TBT.

6.6 Shift Handover

If work is carried over to another shift because the job takes longer than expected and/or
there is a crew shift change, employees and contractors shall make arrangement for shift
handover meetings and a workplace inspection with supervisors/foremen from both shifts to
ensure that the incoming shift is aware of any outstanding jobs, the status of those jobs, and
the status of the plant and any hazards that he or she must be aware of.

Handover items to be covered include:

 TASC form Previous shift reviewed

 Work permit obtained and reviewed (if applicable)

 TBT from previous shift reviewed

6.7 HSSE Notice Boards

Project HSSE notice boards with project specific HSSE information will be provided in
common locations of the site under YAP Enterprises Inc. responsibility. The HSSE
department will keep the information on the boards updated with current information.

6.8 Safety Stand-down

YAP Enterprises Inc. managed employees and contractors shall ensure that learning’s from
all High potential incidents defined through the YAP Enterprises Inc. incident potential
matrix or project incident trends are communicated to the contractor personnel through
stand-down in an effective and efficient manner.

6.9 Bulletins

Incoming alerts will be directed to YAP Enterprises Inc. project HSSE team, who will ensure

That they are distributed to appropriate parties within the project and that copies are
displayed on the project HSSE communication boards.

Employees and Contractors shall be responsible for distributing and communicating all
project related bulletins to their personnel through HSSE Meetings, TBT’s, etc.

Where appropriate, the project will raise project specific HSSE bulletins to capture lessons
learned from project related accidents.

6.9.1 DOCUMENT/ RECORD MANAGEMENT

YAP Enterprises Inc. HSSE project execution plan (Volume 6 of the PMP) and a library of
HSSE and project site-specific programs, procedures and forms will be will be kept
electronically on the project drive and in hard copy in the project site office. They will be made
available for use to all.

YAP Enterprises Inc. and AML and project employees upon request.

Employees and Contractor(s) are to supply YAP Enterprises Inc. with their HSE project
execution plan, procedures, forms and permits for YAP Enterprises Inc. project HSSE dept.
review and approval.

The YAP Enterprises Inc. project HSSE manager is the owner of the project HSSE
execution plan, programs, procedures, and forms and is the only person authorized to make

changes.

Documents and data relating to the YAP Enterprises Inc. managed project HSSE
management system and mining programs, procedures, safe work practices, forms and
permits will be stored in a secure location for a period determined by its relevancy.
The employees and contractors shall keep on file all relevant HSE documentation for the
minimum period of time stipulated by the applicable regulations.

7.0 HSSE MANAGEMENT SYSTEM

The following HSSE Management System hierarchy is adopted for the project:

 Level 1: Project HSSE Execution Plan (volume 6 of the PMP);


 Level 2: Project HSSE Interface Documents;
 Level 3: YAP Enterprises Inc. Generic HSSE programs and procedures;
 Level 4: Project Site-Specific HSSE programs and procedures;
 Level 5: YAP Enterprises Inc. Project Generic Construction programs
and procedures.

YAP Enterprises Inc. HSSE programs and procedures applicable to the mining project are listed
in Appendix 6-3A. Project developed HSSE procedures and forms are listed in

The YAP Enterprises Inc. managed employees and contractors will be required to develop
procedure(s) to manage hazards for their scope of work and to comply with all the
applicable YAP Enterprises Inc. and regulatory health, safety and environmental
requirements as a minimum standard.
Each Employee and Contractor shall create and implement a health and safety plan that
assesses the health and safety risks for the contracted work, and identifies the preventative
measures that will apply when work is in progress. The health and safety plan shall:-

 Be specific to the work;


 Include details of any hazards and risks that other parties could not
reasonably know about;
 Include relevant health and safety rules and procedures;
 Include procedures for dealing with emergencies.
 The project is still at the initial execution stage.

7.2.8 EMERGENCY RESPONSE & READINESS

YAP Enterprises Inc. is committed to ensure the safety of its contractor(s) and employee(s)
and the integrity of its operations wherever they are undertaken.

The below table identifies responsibility of YAP Enterprises Inc. and other parties on the
project in relation to Emergency Response Management (Ref. SHE-PRO-000054).

Emergency Response is YAP Enterprises Inc. controlled mining sites

YAP Enterprises Inc. will assume PRIMACY and has full


responsibility for responding to the Emergency. YAP
Enterprises Inc. will be responsible for mobilizing their own
Category A Emergency Response Teams and notifying YAP Enterprises
Inc. initial contact with YAP Enterprises Inc. will be with the
project manager or his designate responsible for making sure
that emergency arrangements are in place for the activities YAP
Enterprises Inc. are involved in. All media response shall be
managed by YAP Enterprises Inc.

As HSSEMS compliance YAP Enterprises Inc. will integrate and coordinate with AML when

 Accounting for personnel;

 Provision of accurate and timely information to the YAP Enterprises


Inc. Emergency Response Team;

 Supporting emergency response where required;

 Coordination of evacuation of personnel from the site in a safe manner

8.0 MONITORING & REPORTING

The methods used to evaluate the effectiveness of the HSSE controls within the mining
project HSSE Management System are outlined in the following subsections.

8.1 ACTIVE MONITORING

Means of active monitoring will include but not be limited to:

 Inspection of site by a joint team of management, safety and JOH&SC


representatives.
 Observation of work and behavior by first-line supervision/foreman to
assess compliance with site safety rules and procedures;
 Monitoring of performance against project HSSE objectives and targets;
 Management visits incorporating HSSE-related activities; and
 HSSE audits and reviews
 Daily risk assessment monitoring

8.2 REACTIVE MONITORING

The following reactive monitoring tools will be utilized on the project:

IRP (Incident Review Panel) meetings will be held for any recordable or HIPO (high
potential) incidents. YAP Enterprises Inc. and relevant Contractor management will attend
these reviews to ensure that investigation findings are adequate and that resulting actions
support sustainable improvement.

YAP Enterprises Inc. Track wise incident management system shall be used as a tool for
monitoring, tracking and trending all YAP Enterprises Inc. mining project related incidents.

8.3 ACCIDENTS/ INCIDENTS/ NEAR MISSES

All accidents, incidents and near-misses will be reported and investigated and the corrective
actions will be completed within a satisfactory period of time in accordance with YAP
Enterprises Inc. Procedure for accident investigation and reports (Ref. SHE-PRO- 000020)

All injuries shall be reported immediately to project First Aid attendant/medical clinic staff
(AML). In addition, all accidents and incidents causing or not injuries, losses or damages will be
reported to the direct supervisor of the person(s). The Supervisor responsible for the activity or the
area shall notify the YAP Enterprises Inc. HSSE Department. All notifications shall be given
immediately and shall be communicated verbally and in writing by issuing completed Incident
Notification form to the YAP Enterprises Inc. Project HSSE Supervisor or his representative.

YAP Enterprises Inc. Project HSSE Manager or his representative will notify relevant
personnel within the YAP Enterprises Inc. project Management Team.

Project specific incident form (Ref. 164596-HSSE-FOR-001) shall be utilized by all


contractors. The form will be made available to all contractors.

A follow-up system will be used to track all the corrective actions and recommendations
from the accident/incident investigation reports. Actions will only be closed when sufficient
evidence has been received and verified by the YAP Enterprises Inc. Project HSSE Dept.

Internally, YAP Enterprises Inc. will report the incidents/accidents through Track wise,
YAP Enterprises Inc. incident online reporting and tracking tool.

YAP Enterprises Inc. managed contractor(s) and employee(s) shall implement the "Freeze the
Scene Protocol" for all incidents, not just actual or near miss high potential incidents (HIPO),
until such time that approval is given by YAP Enterprises Inc. management that the scene can
be released.

The statutory notification (Liberian) of reportable incidents involving project personnel on


site is the responsibility of YAP Enterprises Inc.
Project incident review panels (IRP) will be undertaken for recordable and high potential
incidents to ensure that investigation findings are adequate and that resulting actions support
sustainable improvement. The relevant employee and Contractor will attend these reviews.

YAP Enterprises Inc. will subject Level 2 incidents, e.g. incidents with high potential to Root
Cause Analyses. YAP Enterprises Inc. A-Z Root Cause Methodology will be used. Selected
members of mining Project Management will be provided A-Z Root Cause Analyses training.

The lessons learned from investigation will be communicated through HSSE meetings,
TBT’s and where necessary stand downs. Project specific Safety bulletins will also be used to
share the lessons learned.

8.4 HSSE ASSURANCE


The following HSSE assurance activities will be undertaken to ensure compliance to the
regulations and conformance to project procedure at the YAP Enterprises Inc. mining
project.

YAP Enterprises Inc. will equally participate and cooperate with all YAP Enterprises Inc.
audits and peer reviews.

8.5 Internal Audits

YAP Enterprises Inc. will establish risk based annual HSSE audit plan (see Appendix 6-5).
The employees and contractors shall have their own audit protocols and schedules.

The audit will be undertaken by YAP Enterprises Inc. to evaluate contractor(s) and
employee(s) health, safety, security and environmental system objectively to determine
whether they conform to the regulatory and YAP Enterprises Inc. HSSE expectations and
whether these arrangements are implemented effectively.

Audits will be conducted by YAP Enterprises Inc. Project HSSE department, along with
members from other departments, where required. Where applicable, an employee and
contractor will be required to develop and implement improvement plan in response to the
audit conclusions.

In addition, relevant third party HSSE audits may be conducted on the project which will
be identified in the project Annual HSSE Audit Plan.

8.6 Site Inspections

Project HSSE inspections will be conducted regularly in accordance with the HSSE
Workplace Inspection Program. These inspections involve the systematic inspection of
Premises, plant, and equipment to ensure the continued effective operation of hardware
controls. Project HSSE inspections will be conducted regularly in accordance with the
procedure (Ref. SHE-PRO-000038 Workplace Inspection Program).

YAP Enterprises Inc., employees and contractors are equally responsible for conducting
workplace inspections and management visits.

8.7 MANAGEMENT VISITS

During the project YAP Enterprises Inc. senior management will undertake site visits to the
project worksites. Visits will be used as opportunities to:

 Conduct HSSE observations;


 Conduct site workplace inspections;
 Address the workforce and demonstrate visible commitment to HSSE;
 Take part in planned HSSE meetings; and
 Follow up issues from previous visits.

Any deficiencies identified during such visits will be discussed with site management, and
Corrective actions will be raised and tracked to a satisfactory closeout. Visit reports will be
issued.

8.8 ACTION TRACKING

A formal actions tracking register will be established to capture and monitor all HSSE related
actions. The status of corrective actions will be reported regularly to YAP Enterprises Inc.
project management team and AML will review during HSSE Steering Committee Meetings.

8.9 AMEC HSSE REPORTING

To monitor performance against project HSSE objectives and targets, a monthly HSSE report
will be compiled by the project HSSE Department. The report will typically include the
following information:

 Progress against lagging and leading indicators;


 Percentage of open and closed HSSE actions
 Areas of concern
 HSSE highlights
 HSSE look ahead

The report will be distributed to YAP Enterprises Inc. project management team, YAP
Enterprises Inc. regional HSSE supervisor and any other relevant parties as established by the
project HSSE supervisor.

9.0 MANAGEMENT REVIEW

A project HSSE management system review meeting will be held twice each year to ensure the
effectiveness of the management systems, joint problem solving, and the development of a
constructive working relationship to enhance and promote HSSE on the project. This meeting
will be attended by the project manager (or designate), HSSE supervisor, heads of departments,
Northgate representatives, and other management personnel, as required.
