1.1 General Description

1.1.1 This statement is to outline the installation of underground services and ducting
manhole at the Package S208. This statement also to ensure the work is carried
out in controlled and systematic manner. All respective parties shall confirm to the
specified requirements, emphasizing, on the aspects of meeting quality standard,
safe working environment and compliance of safety requirements as stipulated in
the Contract during project implementation.

1.6 Work Procedures

1.1.5 Pre-Installation procedure

1.1.5.1 Delivery Plan

a) Data from the site measurement will be referred for material


delivery in accordance with the need and availability.
b) A number of materials will be unloaded at a store yard for
reasonable intervals along the proposed locations installation by
obtaining the approval from local authority.
c) The location where materials will be unloaded must be carefully
selected to avoid any risk for any personnel around that area

d) All the materials shall be approved by SC and PDP prior to delivery


at storage area.
e) All the materials placed at the work areas shall not block any
access.

1.1.5.2 Preliminary & Site Inspection

a) Obtain all related work permit before start work.


b) All materials must be inspected by QA/QC engineer and witnessed
by PDP/SC to ensure all materials are in accordance to required
specifications and approved MSA. RFI to be raised for material
inspection.
c) All works are to be carried out according to the approved MST and
also approved shop drawings/CSD
d) A mock up installation will be carried out if needed prior to start
work.
e) Toolbox briefing should be held before start work. Safety officer
will give safety & environment briefing to all workers and ensure
that they comply with all safety requirements. All personnel are to
be equipped with PPE & safety harness when working at height.

1.1.6 Manhole Installation

1.1.6.1 Excavation

a) To refer with the mapping result done by CWP, piloting shall be


conducted before excavation of trenches. Piloting shall be dug at
positions selected one per sectional length to ascertain the most
suitable position for the work.
b) In no case shall the width of trench excavated be greater than is
necessary for satisfactory execution of the work. The line of the
excavated trench shall be as straight as possible and any bends or
curves must be of the maximum radius possible. The line of trench
shall be marked out. Line enclosing a suitable width of trench shall
be marked by aid of a chalked cord. Location of ducts are to be
encased in concrete (pre-order), the width of trench is dependent
upon the number of pipes to be laid in horizontal direction. In
unstable ground timbering may be necessary to support the trench
shall be no greater than is reasonably necessary for the satisfactory
execution of the work.
c) The depth of trenches shall be such that the average depth below
the surface is according to the respective specification and as
specified in drawings. The floor of the trench must be level and
does not contain any surface irregularities. The bottom of every
trench shall be normally levelled with 25mm of well tamped free
coarse sand.

d) The disposal of water shall be dealt with so as to prevent any risks


of the pipes, cables and manholes to be laid in the trenches being
detrimentally affected. All pumps and appliances required shall be
provided to carry out the necessary pumping and bailing.
e) The manhole position shall be determined by surveyor on site and
witnesses by SC/PDP prior to the excavation works for man hole
installation.
f) All open cut excavation need slope cutting shall not be sleeper than
1 (V) : 1 (H) respectively.
g) For excavation more than 4 metres depth, temporary sheet piles or
similar function of shoring arragnemet to be applied. Sheet pile
confferdam design need to submit seperately with PE
endorsement.
h) Excavation material is to be transported away from work site or
properly sotred at designated area within work site for future use.
i) The excavation is kept drained of water by use of water pumps to a
sediment pond for settlement of silt and discharge to nearby
waterway sily below 100ms/lit.
j) Steps and hand railing shall be installed to provide the safe access
to or egress from exavation pit.

1.1.6.2 Piloting works

1.1.6.2.1 Prior to any excavation works, detection report which was


previously done by WPC to be refered.
1.1.6.2.2 Piloting works to be carried out prior the excavation works to
ensure that there is no existing underground facility under the
excavation area, and if there is, it should be marked and record
accordingly.

1.1.6.2.3 Piloting at ground area, backhoe shall be utlised to excavate to the


depth range from 300mm-500mm at the first lift. The excavated
earth material shall be stockpiled next to the trench for the
subsequent backfill purposes. The above process shall proceed up
to 2.5m deep. The detection and excavation steps shall follow from
step 1 to step 4 as per the figure shown below.

1.1.6.2.4 Excavation work shall be performed in careful manner, with


adequate precautions being taken to prevent or protect any
adjacent structure against settlement and damage.
1.1.6.2.5 An observer will check for signs of services (i.e changes in soil
condition, marker or actual cable or duct surfaves/ sheaths).
1.1.6.2.6 Proceed with manual digging using manpower when notice
changes in condition (i.e spotted sand), discover a concrete
encasement or etc deem necessart to have alert or generate extra
precaution on the trenching works.

1.1.6.2.7 During manual digging work using man power, a careful watch
should be kept for evidence of cables, and repeat checks made
with a locator to determine more precisely the position of any cable
is positively located only when it has been safely exposed, and
even then digging should still proceed with care because there may
be others services or utilities, particularly high voltage cables,
adjacent or lower down.
1.1.6.2.8 When digging run parallel to cables, the cable should be exposed
at appropriate intervals in the immediate vicinity.
1.1.6.2.9 The located services and utilities shall be recorded in the form.

1.1.6.3 Lifting Apparatus

a) When lifting manhole and pipes, make sure the lifting belt length
are long enough to prevent contact with the tongue and groove
area and are kept at appropriate lifting angles. Special precaution
shall be emphasized such that it does not damage the external
coating
b) The manhole lifting apparatus must meet or exceed safe working
load capacity with respect to the lifting points. All lifting points
should be use and the product should be handled with equal
“picking” force on all lifting points.
c) All lifting machine (mobile crane & etc.) must be inspected and
approved prior to lifting.

1.1.6.4 Handling and Installing manholes (CAST IN SITU)

1.1.6.4.1 Reinforcement Bar (rebar) works


a) Rebar will be delivered to site and should be complying with BS
standards requirements. The rebar must be inspected against the
delivery order (DO) and to be properly stored/ stacked, in a row
with the identification and tagging number following the diameter of
the bar.
b) The reinforcement shall be rolled mild steel and hot rolled deformed
high yield bars.
c) Ensure the rebar lapping length is as shown in the construction
drawings.
d) Clear distance between parallel bars in a layer shall not be less
than diameter of the bar or 25mm. Ensure bar spacing is adequate
to allow concrete to flow fully into all areas.

1.1.6.4.2 Formwork Installation


a) Centre line of the manhole to be marked onto the lean-concrete

and dimensions in accordance with shop drawings and fixed the

formwork which has been applied with approved non-staining


releasae agent its internal surface accordinly.
c) All internal formworks are to be cleaned and external formworks
should be properly restrained with struts/ braces.
d) Tie rod will be used to ensure the formwork rigidity is in order and
to avoid the formwork from breaking during concrete pouring
activities.
e) Mark the finish top level of man hole on the installed formworks.
f) Carry out joint inspection with PDP/SC for inspection of the rebar
size, number, spacing, lap length, starter bar and clealiness. This
inspection is to be carried out concurrently with installation
activities. Comments that need to be immediate action are to be
notified to site supervisor in-charge for retification or amendment.
g) Install remaining formwork complete with bracing/strutting/supports
and thses formworks must in perfect verticle, square, tight, rigidd
and able to support load/force from fresh concrete during
concreting activity.
h) Carry out final joint inspection with PDD/SC for installed formworks
and reinforcement bar prior to commencement of concreting works.

1.1.6.4.3 Placing Concrete


a) Concrete shall be placed upon the approval of the PDP/SC. After
the requirment of the formwork and steel reinforcement have been
fully complied.
b) Slump test and temperature test shall be performed before placing
the concrete into the form.
c) Concrete is plaed by either the following methods:
i) By direct discharge, with the use of the chute, and where the
concrete truck can approach the man hole area safely.
ii)By crane, where the concrete truck is unable to approach the man
hole area.
d) Engine driven vibrator shall be used to compact the concrete to
avoid honey comb or segregation.
e) The vibrator should be immersed through the entire depth of the
freshly deposited concrete and into the layer below.
f) Concrete shall not be dropped into space from a height exceeding
2 metres, where trunking or chutes shall be used to avoid
segregation.
g) The pouring concrete activity continud until the concrete reached
the top of man hole area.
h) Concreting shall not be commenced at site if heavy rain
commences prior to arrival of concrete. The concreting team shall
wait until rain subsides prior to ordering of concrete and begin of
the concreting operation.

1.1.6.4.4 Concreting curing


a) Concreting curing of man hole shall be carried out immediately.
b) The exposed concrete surface to be covered with a damp
absorbent material such as gunny sack or canvas or similar
material and ensure that it is always wet.

1.1.6.4.5 Dismantling of Formwork


a) The formwork (side) will be removed no earlier than 12 hours after
the last concrete placement or after the concrete has gained
sufficient strenght. All forms will be stripped in such a manner to
ensure no damage to green concrete. Post concrete inspection will
be carried out.
b) The holes formed by tie rod accessories will be patched up using
approved cement grout materials.
c) Appropriate remedial work will be carried out if defect identified.
1.1.6.4.6 Backfilling works can only be carried out once all the necessary
inspection is carried out and accepted.

1.1.6.5 Handling and Installing Pipe Ducting for open trench method

a) HDPE PN10, HDPE Double Wall Corrugated Pipe, and uPVC pipes
are laid at a minimum depth of 600mm to 1500mm underground.
b) HDPE PN10 will be using electrofusion joint, where HDPE double
wall corrugated pipe and uPVC will be using coupler joint for the
installation works.
c) Sand of 25mm thickness is to be used as a bedding to backfill the
bottom layers of the trench before underground piping laid.
d) After laying first layer of pipes, concrete of 25mm thickness will be
filled on top of the first layer of pipes.
e) Second layer of pipe to be laid after the concreting is done. Once
second layer of pipes laid, earth soil of 25mm thickness will be
filled.
f) Finally, earth soil of 300mm thickness to be backfilled into the
trench fully with original earth soil to original level.
g) All stages of works need to be witnessed to ensure all works are up
to specification.
h) Any excavation works below 4 metres shall be using shoring
method for protection.

1.1.6.6 Handling and Installing Pipe ducting using Horizontal Directional


Drilling (HDD) method

1.1.6.6.1 Drilling Pilot Hole


1.1.6.6.1.1 The HDD drill rig and supporing equipment is set-up at the
drill entry location determined during the pre-site planning
phase.
1.1.6.6.1.2 A pilot hole is drilled along the predetermined drill path.
1.1.6.6.1.3 Periodic readings from a probe situated close to the drill bit
are used to determine the horizontal and vertical
coordinates along the pilot hole in relation to the initial entry
point; the pilot hole path may also be tracked using a
surface monitoring system that determines the down hole
probe location by taking measurements from a surface
point.

1.1.6.6.2 Reaming of the pilot hole


1.1.6.6.2.1 The down-hole assembly is removed from the drill upon
breaking the ground surface at the exit location and is
replaced with a back reamer.
1.1.6.6.2.2 The drill is pulled back through the bore hole and the back
reamer enlarges the diameter of the drill hole.

1.1.6.6.3 Pipe Pull Back


1.1.6.6.3.1 Pipe is the attached, that is slightly longer than the length
of the drill on the exit side of the bore hole.
1.1.6.6.3.2 The pipe is pulled into the exit hole and pull- back
continues until the entire pipe has been pulled into the bore
hole.
1.1.6.6.3.3 Upon sucessful pull-back of the pipe, the drilling equipment
is dismantled and demobilized.
1.1.6.6.3.4 This pipe is then connected to the conventionally laid pipe
and work areas are reinstated.

1.1.6.7 Concreting Manhole Ducting


1.1.6.7.1.1 Grade 20 concrete is used for underground ducting.
1.1.6.7.1.2 Before concrete do the slump test and only continue if the
slump test is paseed or acceptable.

pipes.
1.1.6.7.1.4 Concrete vibrator to be used to avoid honeycomb.

1.1.6.8.2 Then, put the milling cutter in the machine frame, adjusting the
pressure of the milling cutter and cutting two pipe ends. When
received continuous cutting, the cutting pressure will be reduce and
withdrawal the cuter in order to keep the smooth of the pipe end as
in Figure below.

1.1.6.8.3 When the welding plate keep the stable temperature, put in the
machine frame and welding the pipe, following the pipe size and
environment to adjust the welding time and pressure as in figure
below.

1.1.6.8.4 After finished welding, fusion the pipe at the first time, follow the
pipe size to adjust the pressure as in figure 8.

1.1.6.8.5 When the temperature of fusion side reduces as environtment,


move the pipe from the machine frame. The strength of perfect
welding side will be the same as pipe.

1.1.6.9 Setting the Manhole Base

a) Set the manhole base on graded bedding per job specifications


making sure the connectors or pipe openings match design
elevations.
b) Level the manhole base in both directions. Making sure each
additional opening pipe section is plumb before installing the next
opening pipe.
c) In order to allow any differential settlement between manhole and
pipe, a flexible joint incorporating short length rocker pipes should
be constructed as close as possible to the outside of the manhole
or the concrete surround if used. Extra care must be taken to
ensure that joints are properly made.

1.1.6.10 Manhole Jointing

a) Manhole components are provided with joints formed within the wall
section.
b) It is to be sealed with water proof expandable foam duct sealant
with a minimum of 300mm insertion.
c) All duct shall be used up using approved fire& water seal after
mandrel test as stated in IFC IFC : SSP-W-KCSB-STP8-TPPE-

1.1.6.11 Manhole Cover

a) The frame shall be positioned accurately in relation to the cover


before being filled so that the cover is flush and level with the
surrounding carriageway.
b) The frame shall be embedded on a layer of cement mortar. Ensure
that the frame is fully supported along all the four sides. The cover
shall than be placed in the frame.
c) When the cover is correct suited, the edges of the frame and the
cover shall be at the same level. Ensure no voids are left when
filling the frame, the concrete in each pocket shall be thoroughly
temped with reinforcing rods.
d) Finishing shall be done with a trower the concrete in the cover
compartments being left slightly proud but not more than 3.2mm at
the edge to 6.4mm crown in the center of the cover to allow for
shringkage, wear and to protect the metal webs.

1.1.6.12 Backfill

a) Suitable materials free from stones or rocks or other hard materials


shall be carefully spread around the manhole equally to prevent
tipping. Backfill with native soil and to be compacted. Compact the
fill in the same way as the standard trench procedure. The size of
the vibration equipment should be suited for site conditions to avoid
damage to the manhole.
b) Surface debris, trash, worn out or broken tools and equipment must
be removed.
c) For developed areas, surface restoration may include replacement
and compaction of pavement sub-base and base materials and the

paving course. Reconstruction of sidewalks, curbs and gutter that


have been damaged by the contractor operations will be required.
d) For road reinstatement, 300mm thick layer of crusher run shall be
laid and compacted in layer of 150mm. 1 tone roller shall be used
for all compaction.

1.1.6.13 Mandrel Test


1.1.6.13.1 After the pipe is laid, contractor to carried out the mandrel Test to
ensure that the installed pipe is connected from both ends
(manholes).
1.1.6.13.2 A rope will be laid inside the pipe and clothes in the shape of ball
will be tied to the rope.
1.1.6.13.3 Rope will be pulled from one end to the other end and ensure that
the inserted clothes ball can be retrieved. This will show that all the
pipe are connected and it will served as the final cleaning for the
pipe internal before contractor seal it with water resistance foam.

2.0 SAFETY AND HEALTH

2.1 Job Safety and Health Analysis


Safety and health implementation at work site are to be exercised in accordance with Project
Safety Plan Ref. No. SSP-MTDC-STP8-OVPR-PLN-00005.
Hazards, risk and associated control measures associated with activities stated in this MS are
detailed in the Job Safety Analysis (JSA) as per appendix.

2.2 Information to Personnel

 All site personnel must complete and attended the KVMRT SHE Passport Scheme prior to
start work at site. Only personnel who have successfully completed the training for their role
will be allowed working on site.

o Safety helmets, Safety vest, Safety boots, Water Boot, etc.
 Monthly safety meetings shall be held with all the contractors related to the works.
 All workers to attend the weekly toolbox meetings conducted by the safety officer held at site and
attendances shall be recorded.
 Hacking work shall be provided with proper/suitable PPE to the workers.

2.3 Emergency Arrangements


Emergency arrangements at work site are to be exercised I accordance with Emergency
Respond Plan Ref. No. SSP-MTDC-STP8-OVPR-PLN-00006
 Flow chart for the potential emergency such as fire explosion, evacuation, structure
collapse, major injury or fatality from accidents, major chemical spillage, road Accident /
vehicle breakdown, flooding and heavy machinery breakdown, protest/riot, food poisoning
and unstable person has been specified in Emergency Response Plan.

 The organization chart, list of emergency contact person and relevant authorities with their
contact numbers shall be posted on the Notice Board. These lists can be obtained from the
Emergency Response Plan.

 In the event of emergency, the Emergency Coordinator is responsible for co-coordinating


the response strategy and take immediate actions to reduce or eliminate the threats of the
emergency. The SNAH ERT is led by the Emergency Commander and to decide to declare
the “EMERGENCY”. Reference: Emergency Response Plan Rev01 (Item 1.9)

2.4 Legal Requirement


MTD will abide to all legal and other requirements to create a safe and healthy working
environment within it project scope, such as
 Occupational Safety & Health Act 1994
 Factory and Machinery Act 1987 (Safety, Health and Welfare)
 KVMRT Safety & Health requirements
 Local Council (MBSJ) requirements

 etc related to the works

3.0 ENVIRONMENTAL ISSUES

3.1 Implementation and monitoring


All activities at work site shall comply with requirement stated in Environmental Management Plan
Ref. No. SSP-MTDC-STP8-OVPR-PLN-00007

3.2 Environmental Aspects, Impacts & Control Measures


Environmental Aspects, Impacts and control Measures at site shall be complied to General
Specification GS Section G11 and Detailed Environment Impact Assessment (DEIA) Approval
Condition, Environment Management Plan (EMP), Erosion and Sedimentation Plan (ESCP).
The Environment Aspect & Impact Register have been identified and listed in Appendix C

3.3 Erosion and Sedimentation Control

 The control for erosion and sedimentation shall be complied with the Package ESCP, DEIA
Approval condition and GS11.21.3
 Site clearing works shall be limited to construction area only. Earth stockpile area shall be
located away from the surface runoff and protective with sheeting
 Temporary erosion protection for exposed slopes under construction shall be provided until such
time when the permanent measures are provided. Temporary earth slopes that are to be
exposed shall be covered with organic erosion control blanket (ECB)
 Sediment control measure such as check dams/temporary earth drain, silt fences, turfing, silt
traps, wheel washing facility and sediment ponds shall be constructed prior to commencement of
work and shall be maintained.

3.4 Air Quality Control

 Air quality control to be complied with Package ESCP, Package EMP, DEIA Approval Condition
and GS 11.21.1
 Site clearance and earthwork shall be carried out in stages to minimize the exposed soil.
 Vehicle washing facilities shall be provided at site exit. Public roads shall be kept clear of soil and
cleaned
 Water truck shall constantly spray the water to minimize the dust especially during hot weather if
any transport operation going on.
 Traffic control such as speed limit and traffic volume restrictions to be implemented especially at
sensitive areas
 Open burning is prohibited at site

3.5 Noise and Vibration Control

 Noise and vibration control to be complied with Package ESCP, Package EMP, DEIA Approval
Condition and GS 11.21.6
 Movement of heavy vehicles shall be restricted to daytime working hours 8am to 7pm and
minimized during night time, weekends and public holidays. Construction work during night shall
get approval from PDP and local authorities.
 Overall noise levels arising from the movement of heavy vehicles shall be controlled by using
routes that will cause minimum disturbance in the vicinity of the site.
 Noise generating equipment and machinery shall be inspected regularly and maintained in good
working condition. Noise and vibration emitting from equipment shall be placed away from
sensitive receptors.
 All noise generating equipment and machinery shall be shut down when not in used
 Noise level nearby sensitive receptor and human settlement shall be controlled within the limit as
stated below as referred to DEIA approval condition No. 21:

3.6 Water Quality

 Water pollution control to be complied with Package ESCP, Package EMP, DEIA Approval
Condition and GS 11.21.2
 Surface run-off shall be treated prior discharged and shall not exceeds 50mg/L for TSS and 250
NTU for turbidity
 The earthworks activity shall cease if the final silt discharge level exceeds 250 NTU. The
activities can be resumed upon rectification for control measures are done.

3.7 Waste Management

 All type of waste shall be segregated, stored and disposed of in accordance with Package EMP,
DEIA Approval Condition, Waste management Plan and GS G11.21.4

 Spoil, surplus soil, hacked pavement, and demolished concrete walkway will be removed from site
regularly.
 Spoil (from bore piled drilling activities) shall be stockpiled, filtered and treated before being
removed at an approved disposal site.
 Waste containers and storage area shall be sufficient capacity to contain all waste generated
 Waste shall be disposed of at an approved disposal site.
 The approved landfill for domestic waste (municipal waste) is at Tanjung Dua Belas Sanitary
Landfill, Kuala Langat, Sepang while for inert waste is at Dengkil Inert Waste Landfill Lot 557,
Timah Dengkil Langat, Mukim Dengkil, Sepang.
 The approved dumping area for USM material is at Puchong Permai.
 Scheduled waste from maintenance works of machineries, vehicles and tools shall be properly
stored and disposed by DOE licensed contractors at approved facilities. Proper oil tray or drum
shall be placed to prevent the oil spillage all over the place. Subsequent waste shall be collected
and transfer to area as assigned.
 Biomass waste and unsuitable material shall be disposed off at an approved disposal site

3.8 Housekeeping
Housekeeping is an essential part of environmental team for the daily routine. Housekeeping
shall include the following terms:
 Maintain the access/egress road to all working area.
 Store and stack all construction materials in safe and orderly manner.
 Park the machineries in orderly manner.
 No littering around the site.

4.1 Contractual Requirements


Shall be implemented in accordance with submitted comprehensive Project Quality Plan Ref. No.
SSP-MTDC-STP8-OVPR-PLN-00003.

4.2 Inspection and Testing


Shall refer to our Project Quality Plan Section 4.0 Project Operating Policy, Subsection 4.5.1
Inspection and test Status. Please refer to appendix for Inspection & Test Plan (ITP) and for
Control Quality Control Inspection Checklist.

4.3 Follow up and Evaluation


RFIs and respective Permit to Work shall be submitted prior execution of any construction
activities. In additional to this requirement weekly walk-about will be conducted to
measurement, analysis and propose for improvement as construction activities are on-going.
