Dear Sir/Madam:

Please accept this letter as your irrevocable authority to remit my net salary on a FORTNIGHTLY basis to National Commercial
Bank Jamaica Limited (hereinafter called NCB), Constant Spring Branch for deposit to my savings account #334468453
commencing June 2023.

Should I leave your employment for whatever reason, I authorize you to remit all monies owing to me excluding pension
proceeds and amount to cover any debts owed by me to you, to NCB for deposit to the said account. You are also
authorised to advise NCB on termination of my employment with you for whatever reason, should NCB seek verification of my
current employment.

This instruction supersedes previous instructions and cannot be altered in any way without the prior consent of NCB.

Your obligation under this assignment will expire in the following instances:

a. On full repayment of my credit facilities with NCB as confirmed by NCB; or

b. On termination of my employment with you; or
c. On receipt by you of alternative instructions from NCB, whichever is earlier.

It is understood and acknowledged that you are not responsible and shall not be deemed to be responsible for my liabilities to
NCB.

In acknowledgement of receipt of this letter and as an indication that you will comply with the above, kindly sign in the area
designated on this form.
