H.O.P.E MODEL

Introduction

The World Health Organization (WHO) declared the outbreak of coronavirus a 'Pandemic' on
11th March 2020 [1]

The first case in India was reported on 30th January 2020 and since then there has been an
unprecedented impact of this pandemic on our healthcare system. The Crux of the humanitarian
crisis are the underserved and marginalised populations. The pandemic has necessitated that
their needs are to be catered to holistically to avoid long-lasting adverse health outcomes in the
country's future [2].

A brief summary of the public health problems that have arised in the given scenario -

• Relatively low assessment rate of patients suffering with Non-COVID conditions such as
Diabetes, Hypertension, Cancer, TB Mental health disorders, asthma, rheumatoid arthritis etc

• Confusion and lack of proper awareness of the public to properly reach out emergency
services in situations such as road traffic accidents, strokes or heart attacks.

• Stigma and bias against Covid patients in the community

• Lack of knowledge about latest healthcare updates and crucial information

• Lack of awareness or motivation among sections of community to follow safety precautions

Gap in the public and private health sectors.
Solution -OP services
Open OP services- This solution involves usage of open government lands, parks, open
stadiums for a temporary setup of OP services. The advantage of Open OPs is the decreased
possibility of contamination and lesser need for extensive sanitation. This model is inclusive of a
segregation system between suspects and non- suspects, usage of umbrellas for social
distancing among patients following Kerala Model[1], an RTPCR booth, educational games that
bring about behavioral change, broadcast system in waiting area for spreading awareness and
installation of fans between doctors and patients to decrease aerosol spread. It is a cheap and
feasible solution.
Emergency clinics converted from PHCs
• Considering the ongoing initiatives like basthi dawakhanas and
mohalla clinics [3] which have started catering public health needs
similar to PHCs and CHCs, 25% of the existing primary health clinics
(along with other open spaces such as community halls) could be
temporarily converted to emergency clinics

• Standard triage protocol is to be followed to assess patients arriving


at the PHC's [4].

• Live updates on available OTs and beds are sent so ambulances and
information centers for reach.

• Capacity can be built by recruiting volunteer doctors after they


complete a rapid emergency training course.
FEEDBACK CIRCUIT
•Feedback can serve as a powerful tool to improve
and strengthen the current healthcare system and its
performance.
•Collecting helpful information and criticism regarding
currently operating plans will help us improve the
quality of health care services available.
•Taking into account the concerns of patients will help
us bridge the gap between public and private health
sector and make them feel that their opinions are
being valued.

Awareness being the backbone of public health, we came up with a model which caters as:
• Information center
• Building relationship between public and healthcare system
• Chronic diseases in public being answered
• Assurance to public
Location at vegetable markets or grocery stores being common point to connect with maximum
public in the pandemic.
Medical students can volunteer to assist with ideas and running of local booths
BOOTH MODELS
• High population density: 4 booth center with multiple entry and exit points
• Low population density: Single booth

COMPONENTS
LONG TERM
• When digital compliance increases, shifting to web and app based models.
• Drones for surveillance, diagnostic tools.
ACTS OF KINDNESS
• Considering stigma towards COVID19 warriors and patients, act of kindness short stories
of general public are played alternatively with existing monotonous corona ringtone would
have an altering impact on public’s mindset. (6)

Conclusion
The ongoing pandemic has shone complete light on the utility and purpose of the public
healthcare system. It is a crucial time where strengthening and making the best of healthcare
resources will equip citizens to pass through this crisis.
Our solution is an integrated model of little ideas designed to address not only the digitally
equipped population but also the underserved, marginalized masses that are yet to be on par
with the advancing technology. Each little idea can be used individually to reap respective
benefits. The end goal is to bring about a true change and progress no matter how little the
impact might be, after all its the little drops of water that make a mighty ocean.
As our ideas stand on the four pillars of H.O.P.E, we would like to conclude our solution with the
quote
“Where there is life, there is hope”
