AFFIDAVIT OF ONE AND THE SAME PERSON

I, SUHARTO M. OMAR, Filipino, of legal age, married, and a resident of Poblacion

Talayan, Talayan, Maguindanao del Sur, after having been duly sworn in accordance
with law, hereby depose and say:

1. That my name as registered in the Certificate of Live Birth is Suharto M. Omar;

2. That the names KAGI ART OMAR and KART OMAR are both my pseudonym;

3. That the name KAGI ART OMAR was became my pseudonym after my
pilgrimage to Mecca and KART OMAR my pseudonym prior to my pilgrimage;

4. That the discrepancy in the spelling was due to the poor and different utterations
and/or pronunciations of families, friends, and other people who knew me of my
name;

5. That I am executing this affidavit to support the certification issued by the

Barangay Government of Poblacion Talayan in relation to my identity;
