Authorization Letter

To whom it may concern,

My name is Christian Dave E. Roa Graduate of Xavier University Senior Highschool SY:
2018 - 2019 of Grade 12 - Mckeough and I am writing this letter to formally authorize my
Aunt Ruby lyn Mercado to collect my diploma and yearbook on my behalf from Xavier
University Senior High School. I am unable to personally come to the school to collect
these documents due to Me presently living outside the Philippines.
To facilitate the process, I have enclosed a copy of my student ID.
I kindly request that you provide my Aunt Ruby lyn Mercado with the following:
1.) My diploma certificate.
2.) My yearbook for the academic year 2018-2019.
I trust that Ruby lyn Mercado will provide the necessary identification and adhere to all
school/institution policies and procedures when collecting these items. I will take full
responsibility for any actions or decisions made by Aunt while representing me in this
matter.
