CLASSROOM DEMOGRAPHICS DATA COLLECTION TOOL – LEVEL 3

CLASSROOM ARRANGEMENT: How is the classroom arranged? Include seating chart and relevant
information about students important to consider when assigning seating/individual vs. group
seating/partner assignments:

As of right now there are no issues with grouping. Each child is seated next to someone who is not on the
same level as they are. Behavior was also but into account when creating the seating chart.

There are no small groups yet. Everything is still done in a large group setting.

When kids first walk through the door, they pass hooks where their backpacks are hung up.

They are asked to place their binder in buckets at the end of their table and any device they may have in
one of the drawers below.

The class is arranged at five tables with roughly 3 to 4 students at each table.

There is a carpet right in front of the board which is where most of the whole group discussion occurs. To
the side of the carpet is the library and beside that are STEAM games and tools that kids are allowed to play
with when they are given permission.

CLASS DEMOGRAPHICS: Collect information about your students related to the following demographics. Add
additional demographics not listed that are important to consider:

Students eligible for ESE & program eligibility, including the Gifted Program. What accommodations are in their
IEPs/EPs?

UL has an IEP for speech. She gets pulled out of class twice a week. 60 minutes a week to work on her speech.

TI, MA, AL, and AV are part of the gifted program and are pulled everyday for 30 minutes.

Students who have a 504 plan – what accommodations are in their plan(s)?

All students identify has male or female. There are 8 girls and 9 boys.

Socioeconomic status of your students – What percentage and which of your students qualify for free/reduced-
price lunch?

All students can get free lunch at Clark elementary.

The students who are eligible for free lunch are:

BE, ET, EN, JA, JO, RO, KY, and UL

Family demographics – what family structures do your students represent? (i.e., single-parent family, same-
gender parents, intergenerational household, foster or adoptive family, etc.)

Thirteen of the students in the class have a two-parent household with a women and a man.

Two of the students are a part of a single parent family.

UL’s dad is not in contact with the family.

JA dad is in contact but uninvolved in JA’s life.

ACADEMIC DATA – Using diagnostic data to determine students’ instructional needs:

LITERACY (Students in need of RtI support – include student initials)

UL, ET, BI(ELL), JO, and YA- one on one attention is given when needed.

RtI Tier 3:

YA (ELL)- sticky notes in class to help him formulate words.
