AFFIDAVIT OF CANCELLATION WITH NO FINANCIAL OBLIGATION

I, DANNIA MAE L. FIALDE, of legal age, Filipino, and a resident of Block 18

Lot 40 ACM Homes, Sanja Mayor, Tanza, Cavite, 4108 after having been duly
sworn in accordance with law, hereby depose and state:

1. That I am the sole proprietor of PEKS SISG CORNER.

2. That I caused the registration of the said business name with the
Department of Trade and Industry (DTI).

3. That the DTI issued a Certificate of Business Name Registration with

Business Name No. 5324576 dated 09 October 2023 and valid until 09
October 2029.

4. That I have ceased all business operations under the registered business
name and no longer conduct any business activities associated with it.

5. That I hereby submit this Affidavit for the purpose of requesting the
cancellation of the said business name registration certificate.

6. I hereby declare that there are no outstanding financial obligations, debts,

or liabilities related to the registered business name, and all financial
matters have been settled.

7. That I acknowledge that the cancellation of my business name registration

will result in the relinquishment of any rights and privileges associated with
the registered business name.

8. That I am executing this affidavit to attest the truth of the foregoing facts,
and for whatever legal intent and purpose this may serve.
