AFFIDAVIT OF DAMAGES

I, MELVIN TOLENTINO, of legal age, Filipino and resident of 24-U

M. Almeda St., Brgy. Sto. Rosario Silangan, Pateros, Metro Manila, after
having been duly sworn, do hereby depose and say –

1. That I am the authorized driver of that certain motorvehicle –

CHEVROLET with Plate No. ASA 2762 owned by my sister –
Caridad Tolentino.

2. That on November 4, 2023 at around 10:00 am, while I was driving

the aforementioned motorvehicle along Greenwood Executive
Village, when I maneuver along the gate of Greenwood Executive
Village, I accidentally hit the signage post of Greenwood Executive
Village causing scratches and minor dents on the left side (door
panel and body) of the CHEVROLET with Plate No. ASA 2762.

3. That what happened was purely accident and beyond my control

as the authorized driver of the above described motor vehicle on
that day – November 4, 2023.

4. That I am executing this affidavit to attest to the truth of all the

foregoing for all legal intent and purposes specifically for the repair
of the damages caused to the car - CHEVROLET with Plate No.
ASA 2762 .

IN WITNESS WHEREOF, I have hereunto set my hand this

November 7, 2023 at Pateros, Metro Manila.
