JOINT AFFIDAVIT OF TWO DISINTERESTED PERSON

WE, JOANN ORLANDEZ GERMO and RONNIE GINOO GERMO residents of

Barangay Manapa, Buenavista, Agusan del Norte, both legal ages, Filipinos, after
having been duly sworn to in accordance with law, do hereby depose and state;

1. That we personally known RUBELYN MIRABALLES AWA and,

PRESCO CURIBA AWA JR., because we are family friends and we have both
know they for several years;

2. That we also personally known that RUBELYN MIRABALLES AWA

and, PRESCO CURIBA AWA JR., are got married on July 23, 2005 in
Buenavista, Agusan del Norte, based on their Certificate of Marriage License No.
4961215;

3. That we also personally known that Mr. AWA and, Mrs. AWA have
begot Daughter Named CRISHA MIRABALLES AWA born on August 18,
2008 in Butuan City, Agusan del Norte as evidenced of her Local Civel Registry
office of Butuan, City Negative Result issued on August 16, 2023 they do not
have any records of Crisha Miraballes Awa`s Certificate of Live Registration;

3.1. That we are aware of the Awa Spouses have a hectic schedule
at their jobs that time they cause of the delayed registered of their
above mention Daughter;

4. That we are executing herein affidavit to attest to the truth of the

foregoing facts and to support the application for the delayed registration of their
daughter of CRISHA MIRABALLES AWA.

5. That we are executing this affidavit in order to attest to the truth of the
facts aforementioned, and for any legal purpose this may serve.
