AFFIDAVIT OF KINSHIP

I, ARNEL R. LLANEGUIZ, of legal age, Filipino, and a resident of Brgy. Princesa,

Puerto Princesa City, Palawan, after having been duly sworn in accordance with law,
on oath depose and say;

1. That I am the brother of JILLIAN R. LLANIGUEZ, born on November 3, 2006 at

Puerto Princesa City, Palawan;

2. That our parents are already deceased, namely: RUPERTO A. LLANIGUEZ,

died on October 13, 2009, at Quezon, Palawan and VICKY R. LLANIGUEZ
died on July 31, 2017 at Puerto Princesa City, Palawan

3. That JILLIAN R. LLANIGUEZ is 16 years old and still growing and living under
my care and custody;

4. That I am currently requesting for a copy of her Certificate of Live Birth at

Philippine Statistics Authority, Puerto Princesa City, Palawan;

5. That I am the authorized person to secure documents and to sign for and in
her behalf any documents requesting her signature

6. That, I am executing this affidavit to attest to the truth of the foregoing facts
and to comply with the requirements of the Philippine Statistics Authority
for processing of the above said documents, and for whatever legal purpose
the same may serve;
