DAILY DIARY:
As the day started, we arrived at exactly 7:00 am in the morning in the hospital. Then we settled in
an area for us to conduct our lessons and also keep our valuable things. Ma’am Fely start to assigned patient
to us for our actual care of elderly then we proceed to admission area in which we are tasked to listen the
endorsement for the morning shift. I was assigned f with Patient Malayo have a heart problem and
osteoporosis also with a chief complaint of back pain and elevated blood pressure, next is Mrs. Espallardo
with Dx of Congestive Heart Failure and CAP MR with a chief complaint of body weakness and DOB.
After the endorsement I proceed to the patients charting and identify the medication on my shift. Then a I
proceed to the medication area in which I prepare the medications on my shift and administer to my patient.
We take the V/S then also interview the caregiver and the patient for the data needed in our CGA report.
Then after the interview and assessment next work that I’ve done is the charting then transcribed to the
patients charts after our CI checked it and also counter signing the medication and the V/S that I’ve done.
Taking lunch at exactly 12 noon then the preparation to the next event that we will be conducted to the
senior which is titled the “Great Senior Living”. We start by a prayer then followed by health teaching in
which we tacked about the importance of exercise to the seniors. Then we proceed to the recreational
activities like Bingo games, Ball and board games and puzzles etc. also singing and dancing is also
conducted which everyone is actively participated. Then after the activities, we conduct an evaluation on
how the events affects the seniors and care takers, and happily we receive heart-warming and positive feed
backs to the participants After the feedbacking our CI offered us together with the participants with snacks.
Then after the event at exactly 3:30 pm we conduct an open forum to share our ideas and our learnings
during our duty and also the final instruction coming from our CI. Then we prepare ourselves for the
dismissal and at exactly 6 pm we leave the area and call it a productive for us.

INSIGHTS/REALIZATIONS:
• Need to improve my communication skills since this is an important attribute in this profession
• FDAR Charting needs improvement
• Always smile and positive anytime
• Do not alter the data given
• Punctuality and respect are a must
• Attitude must be observed
• Follow the rules and policy
• Recreational activities can greatly help the seniors
• Having that event makes them happy even in a short period time
• Gladly happy to see them smiling
