5- The Operator's employer must ensure that the operator has
suitable medical fitness and technical competencies to drive the plant. This form shows each items which will be inspected by Fleet & HSE personnel of project before entrance on site. The mobile plant will be authorized to enter onto Project Site only if all these items are in good condition. Owner/supplier will be in requested to repair all defective or unsafe parts (HSE department will be not be responsible of loss of time or productivity if this mobile plant is not authorized to enter on site). When all defectives parts are corrected, a permanent or temporary site access pass will be delivered. (A sticker will be display onto the cab windscreen for permanent mobile plant and a cardboard pass will be delivered for temporary mobile plant). The operator has to attend the HSE induction course and receive a permanent or temporary site access badge. The operator must be equipped with all required personal protective equipments. He will have to respect all instructions coming from Project managers, HSE and foremen.
