I, Luzminda Francisco Into, citizens of the Philippines specifically Zamboanga City with a Passport
Registered No. P4997165A currently residing in Kampung Ipil, Jalan Kastam Baru, Lahad Datu, Sabah
wish to collect the new issuance of my passport in possession of your embassy.

In light of the current situation, I cannot be physically there to collect the document(s) as I am residing
in East Malaysia. I would like to authorise my son, whose details are provided below, to collect the
document(s) on my behalf as he currently in West Malaysia. I therefore request you to please allow
for handling over the above mentioned document(s) to the person I authorize.
