By signing this Participation Agreement & Liability Release Form (hereinafter the “Form”), I hereby agree
as follows:
I have been informed of and have understood the various aspects of the Hanoi-Amsterdam
Model United Nations Conference 2023 (hereinafter the “Conference”).
I affirm that I have read, understood, and agreed to adhere to the Conference’s Policy throughout my
participation at the Conference. I understand that if I violate the Policy, I will be solely responsible for any injury
or damage caused, and I may be suspended from the Conference.
I agree to assume all the risks, dangers, and hazards existing in this activity. My participation in this
activity is purely voluntary, and I choose to participate in spite of the risks that the Secretariats cannot
eliminate, including but not limited to the risk of property damage, illness, bodily injury, permanent disability,
and death.
I release the Secretariats from all legal and financial responsibility for any harm that I or my property
might suffer as a result of my participation, even if the harm is caused by the negligence of the Secretariats. I
agree to indemnify and hold the Secretariats harmless from any costs, penalties, legal fees, or judgments
relating to my participation in the Conference.
I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while
participating, or else I agree to bear the costs of such injury or damage myself.
I certify that I am willing to assume the risk of any medical or physical condition(s) I may have. I affirm
that I have completed an official Health Declaration form. In case of serious illness, I authorize the Secretariats to
seek treatment for me on my behalf and agree to indemnify and hold the Secretariats harmless from any costs,
penalties, legal fees, or judgments relating to said treatment.
I am aware that if I fail to follow the instructions in the Conference's Health Declaration Guide before the
event commences, the Secretariats hold the right to deny my access to the venue without a refund. I affirm that
I am willing to be held responsible by health authorities and to release the Secretariats from all legal and
financial responsibility if my participation causes a community spread.
If a court decides that any part of this Form cannot be enforced, I agree to change that part to make it
enforceable. If the unenforceable part cannot legally be changed, it will be severed, but the rest of this Form
will remain in effect.
I hereby certify that I have had the opportunity to read this Form before signing and that the
Secretariats gave satisfactory answers to any questions I had.

For the Participant’s Parent/Legal Guardian, if the Participant is under 18 years of age: I certify that I am the
parent or legal guardian of the above-named participant. I have read and understood this Form in full, and give
my consent for the participant to join the Conference. I agree to be bound by the terms of this Form and will be
legally responsible for the actions of the Participant at the Conference.
