Monday 16th August 2021. I will ensure all safety measures at home and will only send my child to
school if they are healthy and fit.

I also confirm that I will not hold the school responsible for any consequences later in the current
COVID – 19 scenario as the school is following all directives of the SOPs regarding hygiene, physical
distancing, wearing masks, temperature checks etc.

 NWS is following all directives of the Government and has put in place preventative measures to
reduce the spread of COVID-19. However, I understand that it is impossible for NWS to prevent
all risk of COVID-19.
 As a parent, I understand and share my consent with the school for taking my child’s
temperature before entry to the school.
 If any symptoms of COVID-19 are present or become present in the course of the time my child is
on school, I shall arrange for their immediate return from NWS.
 I also confirm that I will not allow my child to come into school if my child:

o has been in quarantine or home isolation in the last 14 days.

o has been in contact with any people who have tested positive for COVID-19 in the past 14
days.
o our area is identified as a containment zone.
o exhibiting any of the COVID-19 related symptoms or have had these symptoms at any point
in the past three days.

 Furthermore, I will inform the school immediately if my child or any member of my household
develops any of the above symptoms after this date.

 I will also inform the school immediately if my child or any member of my household gets
diagnosed with COVID-19 after this date.

 If my child is currently taking any medication or requires any medication in the future, I will
inform the school immediately.

Once in school, I understand and agree that my child must:

o wear a mask at all times.
o maintain 6ft physical distance as far as feasible.
o follow all classroom rules.
o sanitise or wash their hands regularly.
o bring and only use their own equipment and textbooks.
o bring and use only their own water bottle.
o bring and use only their own sanitiser.

Transport to School

I agree to pay the school transport fee for August 2021 for 15 days regardless of the number of days
my child uses it in the month and then the full rate on a monthly basis.

The school will communicate the pickup and drop off times with you in due course.
