LENDING LIBRARY CONTRACT

AGREEMENT

Spring 2013

I, _________________________________ (print name), agree to the following terms in

accordance with borrowing a book from the EOPS Lending Library.

♦ AGREE TO RETURN THE BORROWED BOOK BACK IN GOOD

CONDITION

♦ IF FOR SOME REASON YOU DO NOT NEED THE BOOK, PLEASE

RETURN IT IMMEDIATELY ie, class cancelled, drop the class, etc.

At the end of the semester, you are required to return the book back to the EOPS office.
Failure to do so will result in the following:

1. You will not be eligible to receive your book voucher for the next semester.

2. A hold will be put on your Cuesta account until you return the book in good
condition, or pay the cost of the book to the EOPS office.
